Community Background:

 Briefly describe the situation in the community. You may conduct an ocular
inspection, interview key persons in the community, consult your college
community coordinator and review the community profile. Take note of the

B. Objectives:

General Objective:

State the over-all direction/ long–term objective in the / of the community. (Use
