This is in reply to a letter coming from your office dated December 3, 2015 which we
received on January 8, 2016 informing us of our failure to file the required BIR Form 1601C for
the month of January 2011.

Our records show that we have filed the said BIR Form on February 10, 2011. Relative to
this, we therefore request that our tax records in your office be adjusted accordingly. Attached
are photocopies of our record files for your ready reference.
