Objective:

exercise, fluid and elimination

verbalize understanding of
appropriate interventions to
treat constipation
Long term
-Within the next home visit,
client will report improvement
on defecating with a soft stool.

demonstrate lifestyle changes
to prevent recurrence of the
problem
- Assess the client’s medical,
surgical and social history to
identify conditions associated
with constipation
- Encourage the client for
balanced fiber diet and
supplements
- Promote adequate fluid

limit foods high in fats
- Discuss some over-the-
counter treatment for
hemorrhoids.
- Educate the client about the
importance of a healthy diet,

- A healthy balance diet
consist of fiber helps aid
constipation
- To aid or prevent severe
dehydration cause by
constipation
-To alleviate pain and
prevent further trauma

problem
