AFFIDAVIT OF CONSENT
FOR EXHUMATION AND TRANSFER

WE, EMELITA S. SALVADOR, JOSEFINA S. REBUSTES and MERLA S. MERCADO, all

of legal age, Filipinos, and residents of 168 Ilocos Sur St., Bago Bantay, Quezon City. after
having been duly sworn to in accordance with law, do hereby depose and say:

That we are the surviving siblings of the late Alfredo S. Salvador and Ricardo S. Salvador, who
died on May 13, 2007 in Quezon City and January 7, 2012 in San Jose Del Monte, Bulacan, respectively,
and their remains were buried at Bagbag Public Cemetery, Quezon City;

That we hereby give our full consent for the exhumation and transfer of the remains of the
deceased persons to Providence Memorial Park, San Jose Del Monte, Bulacan;

That we undertake and declare to agree that in the event somebody will question or claims the
remains of our family members, we will be answerable and liable, and further declare that the Quezon
City Health Department will be free from any liability;

That we hereby declare that Bagbag Public Cemetery and its staff shall not be liable to any
person who may present at lawful claim on the above transactions and we undertook to indemnify any
and all persons who shall make a lawful claim on the same;

That we are authorizing our sister MERLA S. MERCADO, as our representative to process and
attend to the necessary documentation of acquiring a permit to your office;

That we are executing this affidavit to attest to the truth of the above statements and this will
serve all legal purposes.

his remainwe are the surviving children of the late Benita C. Benavidez, who died on April 19, 2007 in
Quezon City, and her remains was buried at Baesa Cemetery;

That we agree that the remains of our late Benita C. Benavidez will be exhumed at Baesa
Cemetery and be transferred to Bagbag Cemetery, Quezon City;

That we hereby give our full consent for exhumation of the above-mentioned deceased Benita
C. Benavidez;

That we hereby undertake and declare to agree that in the event somebody will question or
claim the remains of our late mother Benita C. Benavidez, who was buried in the aforesaid cemetery,
we will be answerable and liable and further declare that Quezon City Health Department will be free

his remains was buried at Manila Memorial Park, Holy Cross, Quezon City;

That I hereby give my full consent for exhumation of the above-mentioned deceased Nonilon
Donghit and his bones be buried together with my daughter Stephanie Joy A. Donghit;

That I am authorizing my daughter MILA A. DONGHIT as my representative to process and

attend to the necessary documentation of acquiring a permit to your office;

That I am executing this affidavit to attest to the truth of the above statements and this will
serve all legal purposes.
