I. ACTIVITIES:

The activities I performed in our ward were listening properly to the endorsements done by
the previous shift. Make rounds to the patients and performed a quick assessment and took
their baseline vital signs. While making our rounds, I cleaned the environment of the
patient, stretched their linens and threw their garbage. After, I prepared the medications of
the patient by calculating the right dosage and administered. After that, I documented the
time that the medications were given. I performed care to my patient by doing hygiene
care, giving health teaching to the significant others. During admission, I ushered the
patient with the significant others to their assigned beds, explained to them the rules and
guidelines of the ward and took the initial assessment and informed the physician about the
new patient. During cardiac arrest, I informed the physician and performed CPR. During
patient’s death I performed post mortem care. I Referred patient’s significant others to the
social worker if they cannot afford the patient’s medicines.

II. LEARNING / INSIGHTS:

I learned how to manage my time in the ward; I learned how to prepare patient prior to
surgery. I learned many new cases in the ward, ENT, OB-GYNE, Ophthalmology and
Orthopedics cases.

III. ISSUES / CONCERNS:

My main concern in the ward is the lack of proper ventilation. Patients and their
significant others are complaining about the ward. My second concern is about hospital
equipment. The ward needs ECG machine, Infusion pumps, wheelchairs and bed pan. My
 third concern is that Doctors should not be rude when referred about their patient, A
certain doctor showed unprofessionalism when referred about their patient.

IV. RECOMMENDATIONS:

I strongly recommend that the hospital must provide complete equipment in our ward. Also
the hospital shall provide more budget in the Medicare ward because the patient are paying
for it.
