AFFIDAVIT OF SELF-ADJUDICATION

KNOW ALL MEN BY THESE PRESENTS:

That I, EVELYN S. PATAJO, of legal age, widow, Filipino citizen and a resident of
Luyahan, Pasonanca, Zamboanga City, Philippines, after having been duly sworn to in accordance
with law, depose and say;

That I am the surviving wife of my late husband FELIX B. PATAJO, who died intestate
at Zamboanga Doctor’s Hospital Inc. Veterans Avenue, Zamboanga City on September 06, 2015,
leaving that certain one (1/2) half share in that parcel of land, identified as follows;

“ A parcel of land (Lot 20 Block 9) of the subdivision plan, Psd-09-017685, being a portion
of PCN-09-000018, situated in the Barangay of Luyahan, City of Zamboanga, containing an area
of Eighty (80) square meters more particularly described in Transfer Certificate of Title no. T-
104,987 of the Office of the Register of Deeds of Zamboanga City.”

That I am the sole and only heir of the decedent, being his surviving wife;

That the decedent at the time of his death, left no known debts nor obligation due against
his estate;

WHEREFORE, premises considered and invoking the provisions of Sec. 1 Rule 74 of the
Rules of Court, I do hereby adjudicate unto myself the estate of the decedent, subject to the
liabilities imposed by Sec. 4 Rule 74 of the same Rules of Court in favor of any other heir, creditor
who might be deprived of their lawful participation in the estate of the decedent for a period of
two (02) years from and after the date if registration of this instrument.
