Ambien

* Administer IV piggyback & always on a pump.

USE: Induction of labor


DOSE (milli-units):
* Low Dose: 1-2 mu/hr - increase 1-2 mu q30 mins.
* Regular Dose: 2-4 mu/hr increase 2 mu q30 mins.

USE: To augment labor process.


DOSE: Initiate when contractions are <250 MVU's (montevideo Units) for > 1 hr.
Generally use "low dose" protocol.

USE: Control heavy bleeding after delivery.


Administer:
*IM -10 U
*IV - 20-40 U in 1000 mL IV fluid - run maintenence at 20 mL/hr.

Protaglandins i.e. Cervadil


USE: "Ripen" a cervix as preparation for labor induction.

Administration: vaginally - placed in the prosterior fornix - remains in place for up to 12


hrs.

* Requires continuous fetal monitoring.

* 1 hr must lapse after removal prior to starting Pitocin infusion.

Cytotec (Misoprostol)
USE: "Ripen" cervix.

Administration:
* Vag 25-50 mcg Q6 hrs.
* Oral 25-50 mcg Q6 hrs.

May give up to 4 doses.

USE: control heavy bleeding


Administer: 600-800 mcg PO or PR
*May repeat as needed

Methergine
USE: Control heavy bleeding

Administer: 0.2 mg IM Q 2-5 hrs.


* Do not exceed 5 doses.

Produces a continuous uterine contraction.

*Will raid BP - never give with elevated BP.

Hemabate (Prostin)
USE: Control heavy bleeding

Admin: 250 mcg IM Q 1 1/2 - 2 1/2 hrs

Demerol (Meperidine HCL)

Narcotic
DOSE: 25, 50, 75, 100 mg IM for labor.

PEAK: 30-50 mins.


DURATION: 2-4 hrs
IV DOSE: 12.5 - 25 mg slowly
ONSET: 5 min
PEAK: 20 mins
DURATION: 1-2 hrs

* Fetal CNS depression


* Have to know she won't deliver within 2-3 hrs.

Narcan.

DOSE: IM - 12.5 mg initial dose - 2nd dose repeated in 24 hrs.

Magnesium Sulphate (MgSO4)


CNS depressant.Given to diminish uterine contractions. Pregnancy induced HTN hasa
danger of seizing.

ADMINISTER: 4-6 Gm in 250 mL LR over 30 mins.

Pre-eclampticPt/Seizures
Generally admin. "loading dose" 4 Gm. in 250 mL LR IV over 30 mins. Then 2Gm/ hr

*Monitor MgSo4 levels Q6 hrs. If levels are >9 - stop infusion & notify doc
*Always heck patellar reflexes hourly -stop infusion if reflexes are deminished.
*Antidoet: Calcium Gluconate - IV very slowly (doc give)
