ENROLLMENT AGREEMENT
This is to certify that the parent/guardian whose name and signature appeared below
understands and submits their cooperation and commitment towards the following policy:

1. Students shall wear the proper and appropriate school and P.E. uniform as prescribed in
Student Handbook.

2. Students shall wear at all times in school premises and during official school events and
occasion their official school identification card (ID).

3. Students shall observe punctuality and regularity of attendance. Parents will receive
notice of tardiness and attendance if student commits 5 consecutive tardiness and 3
consecutive absences. Excuse letters are expected from parents to as they ask for
consideration.

4. The school shall not be held responsible for things lost.

5. The physical appearance of students shall be neat, pleasant and presentable. Improper
haircuts, inappropriate hairstyle, unnatural, tinted and/or dyed hair color, untied hair (for
female) are not allowed. Make-up (in any form), nail polish and ear piercing/studs (for
boys) are not allowed.

6. During class hour and as the students stay in school premises, they are banned to use
mobile phones and gadgets. Those will be confiscated by school authorities and will be
returned one week after and through and with parents and guardian.

7. Before class hours and dismissal time, the school will not be held responsible for any
accident and/or any untoward incidents that may happen towards students.

8. Parents are allowed and encouraged for a conference set by the teacher and/or school
administrator/s or requested by the parents in proper venue and place. Thus, parents are
not allowed to enter into classroom during official class hour. Students’ academic status,
behavioral condition and other concern should be discussed with the approval of the
school administrator/s.

9. Collection of money and/or any payment shall be done in the finance office.

10.Parents and guardian are expected to come to school at least during the issuance of cards
or/and in condition the school administrators set a meeting with them.
