______________________ at the BSP Backyard Encampment 2018 to be held at Gamut HNS Ground,
Gamut, Tago, Surigao del Sur on November 16-18, 2018.
I have considered the benefits that my son will derive from his participation in this activity with the
understanding that every precaution is to be taken to ensure his safety in the camp.
I shall held the Camp Staff and the Boys scout of the Philippines responsible for any untoward
accident that may happen beyond their control.
