Reckless Imprudence Resulting in Physical Injury (Motorcycle vs Pedestrian)

Along Provincial Road vicinity of Brgy. San Francisco, Santa Ignacia, Tarlac.

Time reported – 11:45 AM

Vehicle involved Yamaha Mio color black no plate number, driven by Christine Marquez y
Natividad, and resident of Camiling, Tarlac.

John Marvin Salva y Clemente and resident of Santa Ignacia, Tarlac.

Initial investigation conducted shows that prior to the incident, vehicle involved was travelling
said road heading West direction and upon reaching the place of incident allegedly a victim
crossed the road going North direction and accidentally bumped by the said motorcycle. As a
result thereof, the victim sustained physical injuries and was brought to Gilberto Teodoro
Memorial Hospital,Malacampa, Camiling, Tarlac for medical treatment and latter transfer to
Tarlac Provincial Hospital, San Vicente, Tarlac City, Tarlac for Further medication while the
involved vehicle was brought to this office for proper disposition.

Along Mac Arthur Highway, vicinity of Barangay Sta Cruz, Tarlac City

At about 11:00 AM of July 20, 2019

Vehicle 1 – YAMAHA SNIPER without plate number driven by JHERSON HERNANDEZ y

Mercado, resident of Guiguinto, Bulacan.

Vehicle 2 –. TOYOTA VIOS with plate number POO 135, driven by RIZALDY LAWAN y
Tabuyo, resident of Gerona, Tarlac.

Initial investigation disclosed that based on the statement of the witness identified as Gregorie
Lilagan y Tagudin a resident of Gerona, Tarlac, that prior to the incident; Vehicle 1 was
travelling at the south bound lane inner lane of Mac Arthur highway heading south direction
when Vehicle 2 coming from the south bound shoulder lane of same highway abruptly
maneuver U-turn encroaching the lane of Vehicle 1, thereby Vehicle 1 front portion inadvertently
bumped Vehicle 2 left side quarter panel portion. As a result thereof, both vehicles involved
incurred damages of still undetermined amount. After the impact, Vehicle 2 fled towards north
direction but was eventually accosted and turned over to Tarlac CPS PCP 1, Barangay
Salapungan, Tarlac City.

Vehicle1/Driver- RUSI Motorcycle 125 driven by Raymart Pagaduan y Baguio, a resident of

Ramos, Tarlac.

Bicycle/Pedestrian- Carlito Rodolfo y Garay, a resident of Paniqui, Tarlac.

Initial investigation conducted disclosed that prior to the incident vehicle 1 was travelling from
west heading towards east direction, while bicycle was travelling from south to north direction.
Upon reaching the place of occurrence allegedly, bicycle suddenly made an exit at the main
road and accidentally hit the incoming vehicle1. As a result thereof, pedestrian sustained
physical injury and was brought at Paniqui General Hospital while the bicycle and motorcycle
incurred damages of still undetermined amount.

At about 6:15 PM of July 20, 2019 reported at about 6:20 PM of same date.

Honda CRV with plate no XGV 795 driven by Alejo Mendoza Jr. y Daga, a resident of Pura,
Tarlac.

Initial Investigation conducted shows that the Honda CRV traveling aforesaid road going West
direction, upon reaching the place of accident due to several days of down pour of rain the
acacia tree instantaneously fell down directly to the hood of said vehicle. As a result the CRV
incurred still undetermined amount of damages and the electric power line and water pipe line
at said area were cut off or damaged.
