BACKGROUND:
LATAR BELAKANG:

Refer to the occurrence on Monday, 02 September 2019, Aircraft PK-LAQ flight as ID 6832 route
CGK-DTB (Sisingamangaraja XII / Silangit International Airport of North Sumatera), and the
similiar occurrence on Thursday, 12 September 2019, Aircraft PK-LUO as ID 6375 route JOG-CGK
(Soekarno-Hatta International Airport of Cengkareng) where during parking at the apron, it was
reported that the headset cable was pinched on the RH wheel well door.

From the results of this investigation, it was caused when the aircraft departed, both headsets
were still left behind on the nose landing gear leg door.

Mengacu kejadian pada hari Sabtu tanggal 25 May 2019, pesawat PK-LAQ penerbangan ID 6832
rute tujuan CGK – DTB (Sisingamangaraja XII / Silangit International Airport Sumatera Utara),
dan kejadian yang sama pada hari Kamis, 12 September 2019, pesawat PK-LUO penerbangan
ID 6375 rute JOG-CGK (Soekarno-Hatta International Airport Cengkareng) dimana pada saat
pesawat parkir di apron didapati kabel headset juga terjepit pada pintu wheel well sebelah
kanan .

Dari hasil investigasi hal ini disebabkan pada saat pesawat berangkat diketahui kedua headset
tersebut masih tertinggal di nose landing gear leg door.

Regarding with these events which could be a potential hazard related the foreign material or
FOD being carried on the aircraft during flight. Therefore, BAT Safety Department issued a Safety
Notice related with this event stressing to the headset placement when handling aircraft Airbus
A 320.

Terkait dengan kejadian ini yang dapat berpotensi bahaya material asing atau FOD yang
terbawa oleh pesawat pada saat terbang. Oleh karena itu, BAT Safety Department
mengeluarkan Safety Notice terkait dengan kejadian ini yang ditekankan pada penempatan
headset pada saat menangani pesawat Airbus A 320.

Based on background and reference above, BAT Safety Department issued Safety Notice with
stressing to the headset placement when handling aircraft to be followed and implemented as
follows:

1. Place the headset on properly with the jack cable connected with the aircraft headset station
panel.

2. Do not place the headset in a place that is potentially invisible or blocked from the front view
during pushback, for Airbus A320 do not place the headset on behind nose landing gear leg
door.

3. Do not leave the headset on the aircraft without notifying to the standby personnel on the
aircraft.
4. Do not take the headset on the aircraft without notifying to the standby personnel on the
aircraft.
Berdasarkan latar belakang dan referensi diatas BAT Safety Department mengeluarkan Safety
Notice terkait dengan kejadian ini yang ditekankan pada penempatan headset pada saat
menangani pesawat untuk diikuti dan dilaksanakan sebagai berikut:
1. Tempatkan headset dengan benar dengan posisi kabel jack yang terhubung dengan headset
panel di pesawat.
2. Tidak menempatkan headset ditemapt yang berpotensi tidak terlihat atau terhalang
pandangan dari depan pada saat pushback, untuk pesawat Airbus A 320 tidak menempatkan
headset dibelakang nose landing gear leg door.
3. Tidak meninggalkan headset di pesawat tanpa memberitahukan kepada personnel yang
standby atau handling dipesawat tersebut.
4. Tidak mengambil headset dipesawat tanpa memberitahukan kepada personnel yang standby
atau handling dipesawat tersebut.
