AFFIDAVIT OF BUSINESS CLOSURE

I, ANNABELE L. OLIVA, Filipino, of legal age, married and a resident of

Minglanilla, Philippnes, after having been duly sworn to in accordance with law,
hereby depose and say:

1. That I am the legitimate owner of the SARI-SARI STORE located at Public

Market, Poblacion Ward I, Minglanilla, Cebu;

2. That such business has not been operational due to its closure last March 2019;

3. That such business has been closed because I am not operating the business
anymore since above mentioned date;

4. That I am executing this affidavit to attest the truth and for whatever legal
purpose this may serve and specifically for compliance purposes as required by
the Business Permit and Licensing Office of the Municipality of Minglanilla,
Cebu.
