AFFIDAVIT OF UNDERTAKING

I, ARLYN B. MANZANERO of legal age, Filipino, and a resident of

Purok-3, Baan Km. 3, Butuan City, Agusan del Norte, Philippines, after having
been duly sworn to in accordance with law, do hereby depose and state that:

1. I am the same ARLYN B. MANZANERO, an awardee of lot situated at

Blk 14, Lot 14, Brgy. Baan, Km. 3, Bashai Subd. covering an area of 120
square meters;

2. I allowed my younger brother CHRISTOPHER BESERE to occupy my

house/lot for almost 1 year since December 2016;

3. That I will let him stay until December 2018;

4. That I will not allow any other person or group of persons to lease the
entire property awarded to me;

5. That any violation of the foregoing statements and this Affidavit of

undertaking and other agreements made and entered into by and between me
and the City Government of Butuan shall result into the immediate
cancellation/forfeiture and the expulsion of the occupants from the property as
an ultimate consequence; And,

6. That I am executing this Affidavit to attest the truthfulness of the

foregoing facts and circumstances and for whatever legal purpose and intent it
may serve.
