I authorize School of Tomorrow®, Philippines to use photos, or other images of myself

and/or my child (or the child for whom I have legal guardianship) for any promotional
materials regarding School of Tomorrow®, Philippines programs, facilities, or services.
These will not be sold to other parties. Promotional materials bearing these images may
be distributed for free to the public and posted on the School of Tomorrow®, Philip-
pines website. School of Tomorrow®, Philippines reserves the right to use any photo or
likeness for a time period beginning when this form is signed and ending upon written
request of participant, parent or legal guardian.

*Original means that the exhibit/artwork was done/executed by the student. However, the use of existing/commercially available patterns as
basis for exhibits/arts are acceptable – commonly done in needlecraft. Artist may paint/draw by copying from a photograph or any printed
material. Photography should be the sole work (shot) of the student. Downloaded pictures are not acceptable in any photography category.
For Science and Social Studies exhibits, sources – designs, diagrams, collections, research materials – must be properly acknowledged.
