I. IDENTIFYING INFORMATION:
Margie is undergoing peritoneal dialysis 2x a week. The cost range up to (10,000.00 ) ten thousand
pesos a week for life time. She was confined for how many times in different hospitals . Client was
diagnosed with chronic kidney disease stage 5. Client is very much worried how her dialysis sustain
considering that her family is belong to the poor families in their barangay. Thus client came to the
office seeking for possible financial assistance.
IV. BACKGROUND INFORMATION:
A. The Client
Client was born on Feb 13, 1975 at Brgy. Ducligan, Bucloc, Abra. She is the eldest among
the four siblings of mr. and mrs. Parinias Sagudang . Client is a loving wife to her husband and
a responsible mother to her 3 children.She was a former OFW at Israel for almost years.
She came back home when she was not feeling well.Several check up and laboratories
conducted and was diagnosed with chronic kidney disease stage 5.
B. The Family
Walner Bentalan is from Manabo Abra while Margie is from Ducligan, Bucloc, Abra. Both
of them came from a poor families. Walner and Margie engaged and got married. Through
the years, they blessed with 2 kids named Summer Brix and Hezekiah. Margie decided to
went abroad as domestic helper just to help her husband provide the needs and for the
better future of their kids. While Margie is in Israel they were able to construct/ build their
house and they bought a passenger jeepney for an additional income. Client and husband
are helping each other to provide the best for their family until such time that client
diagnosed having a kidney failure.
V. DIAGNOSTIC ASSESSMENT:
Considering the economic status of the family, client is very much in need of immediate
financial assistance to continue her dialysis treatment/ session due to chronic kidney failure /disease
range for almost (30,000.00) thirty thousand a month for life time.
