on Investigation Phase
INTRODUCTION
Investigation is the collection of facts to accomplish a three-fold aim:
The investigation phase provides the police trainees the basic and
fundamental knowledge in investigation, in which in the performance of his duties,
the investigator must seek the six (6) cardinal points of investigation, namely: what
specific offense has been committed, how the offense was committed, who
committed it, where the offense was committed, when it was committed, and why it
was committed.
In fact, this phase helped the Police Trainees familiarize the proper use of
Police blotter and Incident Record Form, make a spot report¸ and prepare an
affidavit of arrest.
ACCOMPLISHMENTS

After 42 days of Investigation Phase, The team was able to accomplish the
objectives of investigation phase by acquiring the fundamental work and knowledge
of an investigator. And the team has accomplished the following;
Know their job as first responders to arrive in the crime scene – Fill-up the
First Responders Form
Prepare the Incident Record Form and Police Blotter.
Make a blotter report and spot report.
Procedure of Booking – Arresting officers prepare the Arrest and Booking
Forms, taking of suspect’s fingerprints, and mugshot
Prepare an affidavit of arrest
OBSERVATION
The following are the observations gathered by the trainees that includes the
observation of the manual, the school, the trainees, and other matters relevant to
training.
The Police Trainees are observed and supervised well by their FTOs.
The FTOs show eagerness in discussing and guiding the PTs on how to
prepare correct reports.
The FTOs are approachable and gives immediate response to the PTs
inquiry.
The Chief Investigator and Personnel of COCPO PS3 are approachable and
share their ideas and knowledge without hesitation that help PTs accomplish
the training objectives of the phase.
The manual does not include the format on how to make an affidavit of
apprehension, affidavit of complaint, and affidavit of witness, and spot report
which is necessary in the learning process of PTs.
ASSESSMENT
The Investigation Phase is a very crucial part in the Field Training Program.
PTs are encouraged to take this phase seriously so PTs can know the correct and
proper procedures in accomplishing the reports needed to support the case filed. It
needs complete, accurate, and meticulous gathering of facts to bring people into
justice.
RECOMMENDATION
Based on the raised issues and concerns, the team highly recommend the
following to further improve and enhance the conduct of FTP:
That PTs has the initiative to ask the Investigation Section in order to learn
how to make the necessary reports (e.g spot report, affidavit of
apprehension) since the Manual does not provide for formats.
PICTORIALS
PSFTP CL 02-2018 TEAM THREE FTX/OJT with their FTOs had their
court observation of the trial of the alleged suspects for the violation of Sec 5
of RA 9165 with presiding judge, Hon Vicente F.B. Rosales at RTC Branch 23
Court Room.
PSFTP CL 02-2018 Team Three together with their FTOs took a pose
with the Lupong Tagapayamapa of Brgy Gusa and with with Hon Imelda
Salcedo, Co-Chair on Women and Children..

PICKPOCKETING SCENARIO
The PTs performed the Illegal Drugs Scenario on December 17, 2019.
The following are accomplished as a requirement:
1. Filled-out IRF
2. Medical Endorsement
3. Filled-out Arrest and Booking Form
4. Receipt/Inventory for Property Seized
5. Mug shots
6. Tenprints of suspect
7. Request for Laboratory Examination for Evidence submitted to PNP
Crime Laboratory
The PTs performed the Illegal Drugs Scenario on December 18, 2019.
The following are accomplished as a requirement:
1. Filled-out IRF
2. Medical Endorsement to JR Borja Medical Center
3. Filled-out Arrest and Booking Form by Arresting Officers
4. Turnover of Property Seized from Arresting Officers to IOC
5. Mug shots
6. Tenprints of suspect
7. Request for Laboratory Examination for Evidence submitted to PNP
Crime Laboratory
8. Blotter Entry
