COMPREHENSIVE AFTER ACTIVITY REPORT

on Investigation Phase

INTRODUCTION
Investigation is the collection of facts to accomplish a three-fold aim:

a) To identify the suspect


b) To locate the suspect; and
c) To provide evidence of his guilt.

The investigation phase provides the police trainees the basic and
fundamental knowledge in investigation, in which in the performance of his duties,
the investigator must seek the six (6) cardinal points of investigation, namely: what
specific offense has been committed, how the offense was committed, who
committed it, where the offense was committed, when it was committed, and why it
was committed.

In fact, this phase helped the Police Trainees familiarize the proper use of
Police blotter and Incident Record Form, make a spot report¸ and prepare an
affidavit of arrest.

ACCOMPLISHMENTS
"Discipline is the bridge between goals and accomplishment"

After 42 days of Investigation Phase, The team was able to accomplish the
objectives of investigation phase by acquiring the fundamental work and knowledge
of an investigator. And the team has accomplished the following;

 Know their job as first responders to arrive in the crime scene – Fill-up the
First Responders Form
 Prepare the Incident Record Form and Police Blotter.
 Make a blotter report and spot report.
 Procedure of Booking – Arresting officers prepare the Arrest and Booking
Forms, taking of suspect’s fingerprints, and mugshot
 Prepare an affidavit of arrest

Other accomplishments related to the phase:

 Fiscal Visitation. Observe the Inquest Proceeding conducted by Fiscal Randy


G Ilisan to the alleged suspect at the Prosecutors Office, Regatta Square,
Pueblo de Oro Township, Upper Carmen, Cagayan de Oro City.

 Visited the Brgy. Lupon at Brgy. Gusa, Brgy Hall.

 Court Visitation at RTC Branch 23 Court Room, HOJ, Goldridge Building,


Mastersons Ave. cor. Macapagal Drive, Upper Carmen, Cagayan de Oro
City, Misamis Oriental. with presiding Judge, Hon Vicente F. B. Rosales.

 Outreach program for the Badjao Community held at Agora PS3


spearheaded by PSSg Cherry C Reyes, WCPD Investigator of COCPO PS3.

 Conducted Ocular Visit at the Tahanan Ng Kabataan (Balay Pag-Asa) of the


City Social Welfare and Development (CSWD) Department at Brgy Lumbia,
Cagayan de Oro City.

OBSERVATION
The following are the observations gathered by the trainees that includes the
observation of the manual, the school, the trainees, and other matters relevant to
training.

 The Police Trainees are observed and supervised well by their FTOs.
 The FTOs show eagerness in discussing and guiding the PTs on how to
prepare correct reports.
 The FTOs are approachable and gives immediate response to the PTs
inquiry.
 The Chief Investigator and Personnel of COCPO PS3 are approachable and
share their ideas and knowledge without hesitation that help PTs accomplish
the training objectives of the phase.

KNOWLEDGE AND SKILLS


Throughout the Investigation Phase, the team learned the different modules
in investigation phase. The team was able to:

 Familiarize the organizational structure of the Investigation Section of


COCPO PS3.
 Learn and understand the basic role of the first responder in CSI.
 Know the concept and importance of Police Blotter and Incident Report Form
(IRF) as the informational record either for Evidentiary or referral purposes.
 Know the different laws under the Revised Penal Code (RPC) and selected
Rules of Court (ROC) which are necessary in the performance of their duties
as police officers.
 Know the cases that fall under the jurisdiction of the Barangay Justice
System.
 Know that only the eight (8) focus crimes is shown in the CIRAS. (Robbery,
Murder, Rape, Carnapping, Motornapping, Homicide, Physical Injuries, Rape)
 Know and Familiarize the 6 Cardinal Rules of Investigation.

ISSUES AND CONCERNS


This are the issues seen by the PTs during the 42 day Investigation Phase.

 The manual does not include the format on how to make an affidavit of
apprehension, affidavit of complaint, and affidavit of witness, and spot report
which is necessary in the learning process of PTs.

ASSESSMENT
The Investigation Phase is a very crucial part in the Field Training Program.
PTs are encouraged to take this phase seriously so PTs can know the correct and
proper procedures in accomplishing the reports needed to support the case filed. It
needs complete, accurate, and meticulous gathering of facts to bring people into
justice.

Therefore, PTs should be keen observer, has mental alertness to catch up


information and initiative to learn by asking questions to station personnel in the
station.

RECOMMENDATION
Based on the raised issues and concerns, the team highly recommend the
following to further improve and enhance the conduct of FTP:

 That PTs has the initiative to ask the Investigation Section in order to learn
how to make the necessary reports (e.g spot report, affidavit of
apprehension) since the Manual does not provide for formats.

PICTORIALS

PCMS Ronnie T Donasco, Chief Investigator PNCO, and PSMS Arlene S


Ferrolino, Admin/ HRDD PCNO, conducted a briefing to the PTs and assigned them
to their respective areas for observation purposes.

At about 02:30 PM to 05:00 PM, PSFTP CL 02-2018 R10 Team Three


FTX/OJT with their FTO observed the Inquest Proceeding conducted by Fiscal
Randy G Ilisan to the alleged suspect at the Prosecutors Office, Regatta Square,
Pueblo de Oro Township, Upper Carmen, Cagayan de Oro City.

PSFTP CL 02-2018 TEAM THREE FTX/OJT with their FTOs had an


Ocular Visit at the Tahanan Ng Kabataan (Balay Pag-Asa) of the CSWD at
Brgy Lumbia, Cagayan de Oro City.
Cherikhawa B Pana, RSW, welcomed the team during their visit.
Frederick O Yap and Maria Marites O Abuzo, CSWD Social Worker, assisted
the team as they tour around the place.
The Tahanan Ng Kabataan, established on year 2007, is the home of
19 Child In Conflict with the Law (CICL). Ergen A Abregana, RSW, is the
Center Head of the said institution.

PSFTP CL 02-2018 TEAM THREE FTX/OJT with their FTOs had their
court observation of the trial of the alleged suspects for the violation of Sec 5
of RA 9165 with presiding judge, Hon Vicente F.B. Rosales at RTC Branch 23
Court Room.

PSFTP CL 02-2018 Team Three together with their FTOs took a pose
with the Lupong Tagapayamapa of Brgy Gusa and with with Hon Imelda
Salcedo, Co-Chair on Women and Children..

At about 09:30 AM to 12:00 NN, PSFTP CL 02-2018 R10 FTX/OJT, led


by their FTOs, assisted the Feeding Program and Giving of School Supplies to
the 65 Badjao beneficiaries at COCPO PS3.
PMAJ JULIUS CLARK PRISCO M MACARIOLA IV, Station
Commander of the said station, headed the distribution of goods to the
beneficiaries.
PSSg Cherry C Reyes, WCPD Investigator of COCPO PS3,
spearheaded the said activity.

SALISI SCENARIO

The PTs performed the SALISI Scenario on December 5, 2019. The


First Responders Form was then prepared as a requirement.

The PTs performed the Pickpocketing Scenario on December 10, 2019.


The Incident Record Form and subsequent Blotter Entry was prepared as a

The PTs performed the Illegal Drugs Scenario on December 17, 2019.
The following are accomplished as a requirement:

5. Mug shots
6. Tenprints of suspect
7. Request for Laboratory Examination for Evidence submitted to PNP
