IMPLEMENTING GUIDELINES:

Employees Under Sick Leave of One Day or More


(As stated: New Policy on Vacation, Sick Leave and Emergency Leave of June 15, 2017)

Sick leaves are paid leave benefit granted to eligible employee to compensate the absences
incurred by such employee who is sick or who has suffered an accident where the illness or injury
sustained is sufficiently serious to disable him/her from reporting to work and discharging his/her
customary duties.

EMPLOYEE ACCESS GUIDELINES: Employees who are on sick leave for one day

1. An employee who tends to rest and avail the sick leave should immediately call one hour
before their designated shift the following:
a. Immediate Superior – This pertains to the company official who the employee
officially and directly reports upon on a day to day or routinely basis.
b. Clinic – The refers to the company clinic of ICTSI. You may inform any of the company
doctors or nurses.
Note: Employee must call BOTH the Clinic and the Immediate Superior. Non-compliance
shall render the Sick Leave without pay.

2. The employee shall call the abovementioned parties and inform them of the employee’s
status. The employee shall identify himself to the abovementioned parties with the
following:
a. Employee Name
b. Employee Number
c. Immediate Superior
d. Work Schedule
e. Illness or Reason for Sick Leave
f. Estimated duration of absence (If sick leave is more than one day)

3. The clinic must inform all affected parties of the employee’s absence due to sickness. This
includes his immediate superiors, supervisor, manager, the Human Resources
Department and the security group. The security group shall block the access card of the
employee who is on sick leave to prevent entrance from the company premises. The
security group must revert to the clinic once the employee access is blocked.

4. The employee is mandated to present himself to the clinic and accomplish a sick leave
application form upon return from a one-day sick leave. The employee shall proceed to
the clinic and submit himself for evaluation. The company doctor or nurse shall examine
the employee and evaluate if the employee is fit to work.
a. If the employee is fit to work – If the employee is declared fit to work, the clinic shall
stamp the employee’s sick leave application form to attest that the employee has
undergone examination and was evaluated fit to work.
b. If the employee is not fit to work – If the employee, upon examination, appears
uncapable of performing his duties and responsibility, shall be sent home and will be
considered as still on sick leave until he is physically well and capable. The employee
shall inform the clinic and his immediate superior on a daily basis of his current status
and the clinic shall also inform all affected parties of the employee’s absence on a
daily basis until the employee is declared fit to work.
c. If the employee is on sick leave for more than one day – The employee shall inform
the clinic and his immediate superior on a daily basis of his current status and the
clinic shall also inform all affected parties of the employee’s absence on a daily basis
until the employee returns to work and presents himself to the clinic. The employee
is mandated to present a medical certificate to the company doctor. The employee
shall undergo physical examination to determine if the employee is capable of
performing his duties and responsibilities. If the employee is deemed capable and fully
recovered, the company doctor shall issue a certificate of fit to work to the employee.

5. The clinic shall inform all parties affected of the absence of the employees return. The
security group shall also reinstate the employee’s access card. The security group shall
revert to the clinic upon reinstatement of access.

6. The employee shall proceed to his immediate superior and submit the sick leave
application form. If the employee was on sick leave for more than one day, the employee
must also present to his immediate superior the certificate of fit to work together with
his sick leave application. Once approved by the immediate superior, the employee shall
submit the sick leave application form to HR Operations for database encoding.
