Canterbury and Coastal CMHT

Job Purpose

To manage a caseload of mental health service users within a Community Mental Health
Team who have varying levels of complexity. This will include assessment, planning,
implementation and evaluation of treatment plans. It will also include robust needs and
risk assessments and a comprehensive approach to the implementation of care and
multi-disciplinary reviews.

This will be in line with the statutory and legal requirements of the NHS care provisions
and the effective implementation of the trust CPA policy and procedure guidelines.

To ensure that resources are targeted and utilised effectively to empower service users
and to safeguard and promote their well being and recovery.

To contribute to the development and maintaining of the clinical and professional


standards which produce good clinical outcomes and meet service objectives.

The post holder will work within the Trust's CPA framework, utilising community
resources and further developing initiatives. There will be an expectation that the post-
holder will ensure user and carer involvement in all aspects of service delivery.

To establish and maintain close and mutually supportive links with colleagues within the
Inpatient/ Psychiatric Liaison/ and CRHT teams. To work closely with / Forensic
Services/ Third Sector Organisations/ and other statutory and non-statutory services.

Job Dimensions

To operate as CPA care coordinator for a caseload of mental health service users with
complex and diverse needs. To undertake comprehensive needs assessments,
developing and reviewing care plan.

To manage and ensure the provision of appropriate medical and psychosocial


interventions to a caseload of service users who experience mental health problems.

To effectively meet the needs of the client group and their carers in line with agreed NHS
and Trust competencies and targets. To ensure that the care offered is evidence based
and in line with NICE guidelines.

To ensure information systems and client records are effectively maintained in


accordance with the Trusts policy.

To input data concerning clinical activity as required ensuring that entries are accurate
and timely.
To ensure that procedures are followed in order for necessary data to be collected (e.g.)
key performance indicators.

To ensure that the views of the service users and carers are sought and incorporated
into treatment plans and that care is sensitive to the needs of all our service users.
To participate in rota’s as required and provide clinical cover for team members in their
absence.

To liaise with CRHTT and inpatient services as required.

To provide written reports, attend Mental Health Review Tribunals and CPA Planning
Meetings, where appropriate, ensuring that key professional input and expertise is
provided.

To communicate effectively with staff, service users and carers. To maximise user and
carer involvement in the delivery of care and treatment. Liaise as required with other
disciplines and organisations to ensure that all issues affecting service user’s care is
managed and communicated to a high standard.

To participate in Mental Health Act Assessments of care coordinated service users as


required. To be responsible for the administration of and or monitoring of medication,
ensuring that all service users receive adequate information about treatment and any
possible adverse effects. Participate in research / audit / clinical governance as
negotiated.

To participate in and contribute to team meetings. To undertake other duties as


appropriate to grade, as requested by the Team Leader.

To participate in the mentoring and supervision of students.

Where negotiated, to attend training courses as required and necessary in order to


compile a portfolio of evidence, supported by experiences in the field to achieve
appropriate competency levels within practice.

To maintain an awareness of legislation, directorate and corporate policies and


procedures, particularly those relating to CPA and Care Coordination in order to ensure
consistency and a high quality of service delivery across the Directorate
