Dartford MODEL Primary Care Network has vacancies for 2 WTE clinical pharmacists.

You will
work across the four member practices to provide a patient-facing clinical pharmacy service. The
PCN has a population of 31,000 and consists of Maple Surgery, Orchard Practice, Dr Shimmins
and Partners (Dartford East Health Centre) and Lowfield Practice.

You will be an experienced pharmacist, with a good understanding of general practice and
primary care prescribing. Experience of working in or closely with primary care will be an
advantage. You may be a non-medical prescriber or prepared to work towards this. You will be
a great team player, but capable of independent, autonomous work. You will have excellent
inter-personal skills and be committed to providing exceptional patient care.

This is a new role and will develop over time, but the expectation is that you will work
autonomously in the multi-disciplinary team in a patient-facing role including face to face and
telephone consultations, supporting patients to manage their long-term conditions effectively
through complex polypharmacy medication reviews. As part of the multi-disciplinary team you
will also provide support to practice staff and patients with regards to prescription and
medication queries, medicines policies and procedures, QOF and local incentive schemes, audit
and medicines safety.

The successful candidates will be required to enrol on the CPPE General Practice Pharmacist
Training Program (unless an equivalent qualification is already held and exemption is agreed by
CPPE).
