1 PURPOSE

This procedure shall be applicable to cleaning stationary atmospheric and low pressure
(up to and including 15 psig) aboveground petroleum storage tanks used in all sectors of the
petroleum and petrochemical industry, including crude oil and gas production refineries,
petrochemical plants, bulk plants and terminals.

This procedure shall provide a guidline for:


- safety hazards to be expected before and during cleaning activities
- how to achieve safe atmospheric condition
- what shall be considered as a safe and not safe atmospheric tank condition
- what to do if safe condition is not achieved or changed from safe to not safe condition
- how to test, monitor and evaluate atmospheric condition before and during tank opening
and entry
- work steps to be followed required before, during and after internal tank cleaning

In specific, this procedure shall be applicable to the internal cleaning activities of tank:
- CP-T-PL-01A, at NC 174 Elephant Field

2 DEFINITIONS AND ABBREVIATIONS

HSE - Health, Safety, Environment


IDLH - Immediately Dangerous to Life or Health
LEL - Lower Explosive Limit

3 RESPONSIBILITIES

3.1 RESPONSIBILITY BY THE OWNER

The owner shall be responsible to decommission, empty and dispose all recoverable product
from the tank.
The owner shall be responsible to provide a tank status safe for tank opening by the
contractor.
The owner shall have the overall authority that at all time the safety to personnel and plant
equipment is provided.
In case that the owner will notice that a safety hazard is existing he shall have the right to stop
any activities by the contractor and freeze all activities until safe work conditions are provided.

3.2 RESPONSIBILITY BY THE CONTRACTOR

The contractor shall be responsible to furnish all manpower and equipment required to clean
the tank internal and to dispose all liquids contaminated with hydrocarbons resulting from
the cleaning process to a disposal point designated by the owner.
The contractor shall be responsible to follow all safety aspects as outlined by this procedure
and the owners provided HSE regulation if any.
The contractor shall be responsible to ensure together with the owner that at all time the
safety of personnel and plant and equipment is provided for.
The contractor shall be responsible to follow the owners provided work permit system and
confined space entry permit.
The contractors SM is responsible for the implemention of this procedure.
The contractors QA/QC Manager or his designee shall be responsible for the review of the
QA/QC records.
The contractors designated construction SV shall be responsible to carry out the works as per
this procedure and applicable standards and the compliance of his work force in respect to the
safety precautions.
The contractors HSE specialist further named as the Entry Supervisor shall be responsible
for determining the requirements and whether or not acceptable entry conditions exists at time
of entry
A permit is required for confined space work , where entry is requiered.

4 STORAGE TANK HAZARDS

4.1 TANK HAZARDS


All aboveground, atmospheric, low pressure storage tanks that have contained crude
petroleum, petroleum products, additives, sludge, or residue have the potential to present one
or more of the following hazards during some phases of the tank decommissioning, isolation,
preparation, entry, testing, inspection, cleaning, repair and recommissioning operations:
Limits for hazardous tank atmospheres are provided under para 5 of this procedure.
Reaching one of the listed limits shall result in not permitting any personnel to enter the tank
without appropriate protection equipment or immediate work stoppage and vacating tank
by any personnel in tank.

4.2 Oxygen Deficiency, Workers entering oxygen deficient atmospheres in tanks are subject to
potential hazard e.g. exposure to toxic substances or asphyxiation.
The normal oxygen content of fresh air shall be noted as 20.9 % by volume of air.
If the atmoshere contains less than 19.5 % oxygen by volume air the atmosphere shall be
considered having a oxygen deficiency.
If the atmoshere contains less than 16 % oxygen by volume air the atmosphere shall be
considered being IDLH.

4.3 Oxygen Enrichment, Workers entering an oxygen-enriched atmosphere in a tank are subject
to increased risk of fire and explosion due to the increased flammability range of vapors and
gases in the tank.
If the atmosphere contains 23.5 % or more oxygen by volume air the atmosphere shall be
considered having a oxygen enrichment.

4.4 Fires and Explosion, Vapors from liquid hydrocarbons are heavier than air and displace the
air in a tank.
Prior to vapor and gas freeing, degassing and ventilation, the atmosphere in the tank will
normally be stratified, with the bottom portion of the atmosphere having the lowest oxygen
concentration, the middle portion having additional oxygen and the upper portion having the
highest concentration of oxygen.
Every hydrocarbon has a minimum concentration of vapor-in-air called the lower explosive
limit (LEL) below which a fire will not occure on contact with a source of ignition, to lean to
burn.
There is also a maximum concentration of vapor-in-air called the upper explosive limit (UEL),
above which a fire will not occur, too rich to burn.
The range between LEL and UEL is called the flammable range.
During ventilation and gas freeing the original vapor rich atmosphere will be diluted with air
and the mixture will enter the explosive range.
Special care and the removal of any source of ignition must be taken at the moment and
location of:
- Tank entry manhole during initial opening and start of ventilation
- Vent nozzle at tank roof during before, during and after venting
- All other tank openings if any allowed
After the tank has been emptied and freed of vapor, flammable vapors may be emitted from
any but not limited to of the following:
- remaining product at hollow roof supports, sumps, any tank attachments such as
sample lines,level gauges, nozzles and attached piping.
- sludge, scale
- inadvertent entry of liquid or vapor from outside sources from open connection lines
leaks and spills in the vicinity of the tank or vapors emitted from such spills.
- flammable vapors may remain in areas of the tank due to stratification or channeling.
- after gas freeing, ventilation and initial cleaning from remainig sludge which has been
agitated by the cleaning personnel during final cleaning.
- oils soaked clothes or cleaning cloths
- dust
Ignition sources can be but not limited to:
- open flames
- internal combustion engines
- welding, cutting other hot work operations
- lightning
- smoking
- cigarete lighters carried by working personnel
- non-explosion proof electrical equipment as radios, cell phones, pagers
- electric short circuit caused by worn or defective extension cords
- non-approved electric lamps, flashlights, power tools, electric fixtures and switches.

4.5 Toxic substances, exposure to toxic substances can result in irritation, injury, acute or
delayed illness or death, depending on the charcterstics of the substance, their nature
and concentration and duration of exposure.
They can affect either the tissue at the point of contact or organs remote from the point of
contact.

Toxic substances can be categorized as following:


- Irritants
- Corrosives
- Acutely toxic
- Chronically toxic

The presence of the following toxic substances shall be considered to be present for this
scope of work, namely cleaning of CP-T-PL-01A at NC 174 Elephant Field GOSP:

- Little concentration of H2S

5 LIMITS FOR HAZARDOUS TANK ATMOSPHERE

5.1 Oxygen Limits

The permissible Oxigen Limits shall be as following:


- oxigen deficiency : 19.5 % oxigen per volume of air
- oxigen enrichment: 23.5 % oxigen per volume of air
No tank entrance shall be permitted without appropriate respiratory equipment if the tank
atmosphere contains less than 19.5 % oxygen per volume of air.
No tank entry shall be permitted if the tank atmosphere contains 23.5 % oxygen per
volume of air or above.
If during the works the tank atmosphere will reach one of the above limits all work shall stop
immediately and all entrants shall vacate from the tank.

5.2 Flammable vapor-in-air Limits

- 10 % of LEL (Lower Explosive Limit)


No tank entry shall be permitted if the vapor-in-air level in the tank atmosphere are
above 10% LEL.
If during the works the tank atmosphere will reach the above limits all works shall stop
immediately and all entrants shall vacate from the tank.

5.3 Toxic Substances Limits (H2S)

The permissible exposure limit (PEL) for hydrogen sulfide (H2S) as per OSHA shall be:
- 10 parts per million (ppm) in a 8-hours workshift
- 15 parts per million (ppm) for short term exposure averaged over 15 minutes.
No tank entry shall be permitted without appropriate respiratory equipment if the tank
atmosphere contains more than 10 ppm of H2S.
If during the works the tank atmosphere will reach the 10 ppm limit all works shall stop
immediately and all entrants shall vacate from the tank.

6 SAFETY PERSONNEL PROVIDED BY THE CONTRACTOR

The contractor shall provide the following safety personnel not directly involved in the tank
cleaning activities:

6.1 Entry Supervisor (one per work shift)

Entry Supervisor shall authorize entry, oversee operations and terminate entry as required by
the permit or conditions.
Conditions where tank entry can be permitted and where tank entry shall be prohibited or any
entrant shall vacuate the tank internal is described under para 5 of this procedure.
Entry Supervisor shall cordinate and monitor any emergency action as detailed in attached
Emergency Action Plan.
Entry Supervisor shall ensure that no employee is carrying a cigarette lighter or other ignition
source with him.

6.2 Medical Responder (one per work shift)

The Medical Responder personnel shall be responsible to provide first aid to any injured
personnel.

6.3 Rescue Team (two per work shift)

The Rescue Team shall be responsible to rescue entrants from the permit required confined
space.
At least two number of personnel not involved in entering the inside of the tank, but which can
be assigned to supporting works outside the tank.
Above designated personnel must be fully aware about the possible hazard in the over-all
activities and must be sufficiently trained to fullfill their tasks.

7 TESTING TANK ATMOSPHERE

7.1 BEFORE ENTRY

Before workers enter the tank at the beginning of each workday or shift, the entry supervisor
shall assure that tank´s atmosphere is tested and evaluated for oxygen, flammable vapors
and toxic exposures and inspected for physical and other hazards.
After opening the entry manhole Entry Supervisor shall conduct the atmospheric tests as
detailed under this para, first reading to be taken directly outside of the manhole.
Upon evaluating the results using the limits as listed under para 5 of this procedure the Entry
Supervisor shall decide if further investigation of the tank internal can be permitted or if the
entry shall be prohibited.
If the results will permit the tank entry the Entry Supervisor shall carry out a representative
investigation of the entire tank area, having a life line attached to him.
Entry Supervisor shall slowly enter the tank continiously measuring the tank internal
atmosphere as listed below under this paragraph.

In case of any limits as listed under para 5 of this procedure has been reached at any of
the detected location, the Entry Supervisor shall vacate from the tank and further cleaning
or ventilation shall be carried out from the outside of the tank.

The entry supervisor shall issue entry permits provided that all entry requirements are met.

When reentering a tank following an evacuation or a break in entry, the tank´s atmosphere
shall be retested and re-evaluated again to determine that conditions and entry requirements
are still being met.
A sample of the entry permit to be posted at the tank´s entry manhole is attached to this
procedure.

7.2 DURING ENTRY

The entry supervisor shall continiously test the tank´s atmosphere for oxygen levels,
flammable vapors and toxic exposures, while entrants are inside the tank, to ensure that
desired atmospheric conditions are maintained in accordance with the requirements detailed
under para 5 of this procedure.
In case of any limits as described under para 5 of this procedure are reached, all works shall
be stopped immediately and all workers shall leave the tank and evacute to a designated
area.
All machines shall be switched off and put into safe condition.
The entry permit shall be withdrawn and the tank shall be marked as prohibited for entry by
posting a signs next to the entry manway and safty barrier tape to be fixed across entrance.

7.3 TESTING SEQUENCE

Most combustible gas indicators work on a design principle that depends on the presence of
adequate oxygen level to obtain accurate reading.
For this reason, initial and subsequent tests shall be conducted in the following order:
- oxygen content
- flammable vapors
- toxic exposure

8 ISOLATING THE TANK

8.1 ISOLATING LINES

After decommissioning Owner and Contractor shall jointly assure that the tank is isolated
from all lines connected to the tank, including but not limited to:
- product lines
- vapor lines
- relief lines
- gauging lines
- water draw off lines

All tank attachements e.g. level gauges and sample lines shall be drained and the closure
valves to the tank shell shall be closed.

Lines shall be isolated as close to the tank as possible by installing blanks between two
flanges using two gaskets and fully engaged bolts or stud bolts in all flange bolt-holes.
Double block and bleed valves with lockout/tagout devices may alaso be approved to isolate
lines.
Lines disconnected from the tank shall have blinds installed across the opening, using gaskets
and fully engaged bolts in all flange bolt-holes.
Method and location of isolating devices shall be indicated on attached P&ID.

8.2 ISOLATING ENERGY SOURCES

After decommissioning Owner and Contractor shall jointly assure that all energy sources
tank equipment and auxillaries have been disconnected and isolated, including but not limited
to:
- all electrical counsumers
- all instruments and sensors
- all hydraulic and mechanical equipment, if any

All power supply cables shall be completely disconnected at the closest main junction box
adjacent to the tank.
Method and location of power supply cables and power supply cables to instruments and
sensors to be disconnected shall be indicated on attached cable schedules, single line
diagrams and junction box layouts.

All other energy lines, if any like hydraulic lines for tank equipment and auxiliarys shall be
disconnected and blanked.
Hydraulic equipment shall be made pressure less.
In case of CP-T-PL-01A no such equipment exists.

8.3 FOAM LINES

If the tank has a fire protection foam system the lines shall remain connected throughout the
tank cleaning activities for the event of fire within the tank.

8.4 CATHODIC PROTECTION

If a tank or lines have cathodic protection, the owner shall assure that whenever a tank valve
or line is to be disconnected, safe isolation practise shall be followed, in the order as shown
below:
- turn off the cathodic protection system
- install a bond wire from the tank to the lines
- disconnect and remove the valve or line
- remove the bond wire only after the valve or line is disconnected

In case of CP-T-PL-01A no cathodic protection system exists.

9 PERSONNEL PROTECTION EQUIPMENT

Each of the tank entrants shall be equipt with the following Personnel Protective Equipment:
- negative pressure respirator
- safety harness
- safety rubber boots
- hard hat
- hand gloves
- working clothes covering entire body
- safety glasses
- safety goggles
- two life lines long enough to reach any internal work location of the tank, placed at the
location of the tank entry manhole

### OTHER PROTECTIVE AND FIRST AID EQUIPMENT

The following minimum protective equipment shall be available at the work area:
- two fire extinguisher one at entry manhole, one inside the tank close to the work area
- first aid kit
- oxygen kit for first aid purposes.
- eye wash equipment.
- standby ambulance car

11 ATMOSPHERIC TESTING EQUIPMENT

Entry Supervisor shall be equipt with the following testing equipment:


- oxygen meter
adjusted in fresh air to indicate 21 % oxygen
- combustible gas indicator
with direct reading that measure flammable vapor-in-air concentration as a percentage
of the LEL, with 100 % being the beginning of the LEL
- toxic substance indicator
in case of CP-T-PL-01A to indicate H2S
- or an combination type instrument
### TANK VENTILATION

12.1 VENTILATION
After the tank has been decommissioned and the all lines has been isolated as stated under
para 8 of this procedure a compressed air operated air blower shall be installed at:

- shell nozzle: MW-1, 24" shell manway

Reference shall be made to: GS-MM-M-DD-15-001 attached to this procedure.

12.2 INITIAL VENTILATION

The entry supervisor shall check the decommissioning and isolation status of the
tank prior to starting any of the listed activities below.
He shall follow and check, the applicable paragraphs as indicated on the entry permit.
The entry supervisor shall assure that the postings required and as stated under para 4 of
the emergency action plan have been implemented.
for locations of required postings refer to: GS-MO-P-LA-00-003 attached to this procedure

Open the following roof nozzle:


- N-33, 24" Emergency Vent Manhole and secure by chain or wire to stay in open position.
- MW-3, 24" Roof Manway, remove blind flange and store on platform
For locations of roof manholes refer to: GS-MM-DD-15-001 attached to this procedure.

Open shell manhole: MW-1, 24" shell manway, carfully to avoid hard metal to metal contact
during removal non-sparking tools shall be used.
For locations of shell manhole refer to: GS-MM-DD-15-001 attached to this procedure.

Employess working directly at the shell manhole shall be equipt with the PPE as detailed
under para 9 of this procedure.
Only non-sparking tools shall be allowed during removing of any tank attachements.

After removal of the shell manhole cover, the air blower shall be attached to the shell nozzle
the air blower shall be electrically bonded to the tank to prevent static sparks.
The bonding wire shall be equipt with cable lugs at both ends and shall be rigid connected to
the shell and air blower body by using hexagonal bolts.
The bonding wire shall be 5m long and shall be connected to the air blower body and tank
shell prior to the actual installation of the air blower to the shell nozzle.

After the air blower has been connected to the tank shell nozzle and it has been assured that
the air compressor has been earthed, the blow hose shall be connected to the air blower and
the compressor shall be started.

The tank shall be vented for a minimum period of 6 hours, prior to removing the air blower
and initial cleaning from outside the tank can start.
The air blower shall be removed clear of the tank before the bonding cable is removed
and the air blower shall be placed clear of any emergency exit points.

During the initial stages of vapor and gas freeing, while the tank contains high concentration
of flammable vapor and gas , the Entry Supervisor shall restrict all works in the immediate
area surrounding the tank and on top of the tank.
Continious atmospheric testing shall be carried out downstream of the current wind direction
to monitor potential hazard inside the existing plant facitlities.
If any hazardous atmosphere has been detected by the Entry Supervisor, all personnel shall
stop work and vacate from the area as directed by the Entry Supervisor.

### INITIAL TANK CLEANING

Upon ventilation of the tank atmosphere for the minimum period as stated under para 12 of
this procedure, the Entry Supervisor shall determine the pontential hazard and exposure as
described under para 7 of this procedure at the outside of the entry manhole.
If the results indicating that the requirements of para 5 of this procedure being met at the
the outside of the tank the designated workers can start cleaning the tank from outside the
tank opening.
The Entry Supervisor shall carefully check about any ignition source carried by the workers or
or in the vicinity of the tank manhole, e.g. cigarette lighter or any of the possible sources but
not limited to the equipment as listed under para 4 of this procedure.
Employess working directly at the shell manhole shall be equipt with the PPE as detailed
under para 9 of this procedure.

The workers shall start flushing the tank wall with water, provided by a 2" fire hose connected
to the local fire main.
The oil remaning on the internal tank wall shall be flushed down from the upper parts of the
inner tank shell towards the the tank bottom.
Once the inner tank shell has been washed, all sludge and washed oil shall be flushed by
using the water stream of the fire hose towards the tank sump.
During the washing process the water flushing to the tank sump shall be sucked out by the
suction truck having a suction line attached to the drain line as indicated on attached

drawings: GS-MO-P-LA-00-003, GS-MO-P-GA-00-057 and GS-MO-P-GA-00-040

After initial cleaning the Entry Supervisor shall check the tank atmosphere as described under
para 7 of this procedure and shall decide if tank entry shall be permitted or further washing
shall be carried out.
Contaminated water resulting from the washing process shall be discharged to the open drain
pit next to closed drain tank and then pumped into the closed drain tank CP-T-DO-01.
Expected volume of contaminated water shall be assumed being 50 m³ (315 bbl).

### FINAL CLEANING


After initial cleaning has been carried out the following procedure shall be followed for final
cleaning:

Instali air blower at MW-1 as described under para 12 of this procedure and ventilate the tank
atmosphere for a minimum period of 30 minutes.

After above ventilation period open clean out door: MW-2, 36"x48" flush type clean out door.
For locations of clean out door refer to: GS-MM-DD-15-001 attached to this procedure.

The Entry Supervisor shall test and evaluate the tank atmosphere as described under para 7
of this procedure.

If the Entry Supervisor has permitted the tank entry the entrants shall start to flush remaining

residues on the tank shell by using the water stream from the fire hose towards the sump,

After the tank shell has been washed down the bottom of the tank shall be flushed using the
water stream from the fire hose towards the sump.

While the tank shell and the tank bottom is being treated with the water stream, one entrant
shall wipe the treated tank surface with a broom to support the cleaning and washing process.

Prior start of water flushing activities a wooden barrier minimum 350 mm high shall be fixed to
the bottom of the flush type clean out door to serve as a barrier and to avoid contaminated
water flooding out from the tank.

The entry supervisor shall continiously testing the tank´s atmosphere for oxygen levels,
flammable vapors and toxic exposures, while entrants are inside the tank, to ensure that
desired atmospheric conditions are maintained in accordance with the requirements detailed
under para 5 of this procedure.

Employess entering the tank shall be equipt with the PPE as detailed under para 9 of this
procedure.

Contaminated water resulting from the washing process shall be discharged to the open drain
pit next to closed drain tank and then pumped into the closed drain tank CP-T-DO-01.

After the tank shell and the bottom has been treated with the water stream from the fire hose
the same washing procedure using a power washer shall be repeated.
The power washer shall be installed outside the tank at least 5m next to the entrance manhole.

During the entire washing process having entrants inside the tank the air blower shall be kept
in operation to provide sufficient ventilation to the tank atmosphere.

After the power wash of the internal tank surface, the remaining water shall be piushed towards
the sump by using brooms and rubber squeegees

Once the water has been removed the tank bottom and the tank shell if required shall receive
a diesel treatment by spraying diesel onto the tank tank surface.
The diesel shall be sprayed onto the tank surface by using a hand pressurized spray container.

The diesel film shall be allowed to soak over night and the following day the diesel shall be
washed away by using the power wash equipment as described above.

In the event of fire, explosion, leak, or other emergency necessitating the evacuation of employees, the following procedure

to wind direction or other circumstances, at which time you should proceed to secondary refuge area.

3.1.1 Initial Refuge Area : Outside Bund Wall Area of CP-T-PL-01A, East side at FW Hydrant

3.1.2 Secondary Refuge Area : Muster Point of AGIP infront of Control Room, North Side

5. EMPLOYEES RESCUE ACTIVITIES

5.1 Entry Supervisor will coordinate rescue team, check PPE of rescue team and inform about potential hazard.
5.2 Upon direction by Entry Supervisor first rescue personnel will enter tank and attach lifeline to downed worker.

5.3 Second rescue personnel outside tank will pull out downed worker with support of first rescue personnel,
if first rescue personnel shall not leave tank internal, upon direction of Entry Supervisor.

5.4 Rescue Team will safely and rapidly decontaminate downed worker

5.5 Medical Responder will initiate Basic Life Support on downed worker

5.6 Following transfer of affected worker, the Entry Supervisor will debrief the team and conduct an investigation

5.7 Following the investigation, the Entry Supervisor will submit a written report to the owner and to own manager

This Emergency Plan shall be read and used in conjunction with GS-MO-Q-PR-00-065.
