AFFIDAVIT OF LOSS

I, TERESA LONZAGA DE LEON, Filipino, of legal age, married, and a

resident of Mabini 5th street, Digos City, Davao del Sur, Philippines, after
being duly sworn to in accordance with law, hereby depose and state:

That I am member of the Social Security System;

That as such, I was issued an identification card with SSS ID No. 09-
1974829-1;

That I always bring with me the said identification card and I usually
place the same in my wallet;

That sometime in November 2019, while at home, I realized that my

wallet was lost;

That despite diligent search and efforts to locate the said wallet, and
my SSS ID, I could not find them such that I now believe that they are now
lost beyond recovery;

As such, I am executing this Affidavit of Loss to attest to the

truth of the foregoing and to support my application for the issuance of a
new SSS id, in lieu of the one that was lost.

single, and a resident of Mabini 5th street, Digos City, Davao del Sur,
Philippines, after being duly sworn to in accordance with law, hereby depose
and state:

That I am student of Cor Jesu College;

That as such, I was issued a school ID by Cor Jesu College;

That I always bring with me the said school ID and I usually place the
same in my bag;

That sometime in November 2019, while at home, I realized that my

school ID was lost;

That despite diligent search and efforts to locate the said school ID, I
could not find it such that I now believe that it is now lost beyond recovery;

As such, I am executing this Affidavit of Loss to attest to the

truth of the foregoing and for any legal purpose this affidavit may serve.
