ANESTHESIOLOGY 260

Endorsement

I. POSTS
A. DUTY AM or PACU 1 & 2
• 7am to 7pm
• Call time: 7am, sign at the attendance logbook that can be found
at the Anesth Residents’ Lounge (kulay blue na log book)
• 2 interns during the weekdays, only 1 intern during the weekend
• IOD is in-charge of receiving all the post-op patients and assessing
them if they are already ready for ward transfer. When a patient is
wheeled in from the OR, get a copy of the form below and fill it up
while Anesth in charge of the said patient shares to you the details
of the operation. Use this form below (ask a copy from the lerler
secretary Maam Erna):
• Insert this on the monitoring clipboard of the patient.
• (Pagod nako magEnglish) Techinically, hindi ikaw yung
nagmomonitor sa patient. May kanya kanyang nurse in charge sila.
Sisilip-silipin mo lang yung vital signs ng patient from time to time.
Kung may abnormalities (usually high BP), assess mo at alamin mo
kung ano yung etiology (usually post op pain). If may problematic
patient, refer mo yung problem sa PACU ROD/Anesth-On-Duty
para makapagbigay kayo ng intervention.
• Lahat ng referrals mo, i-log mo dito sa form na to (hingi na lang
kayo kay Maam Erna):

• Wala namang definite number of hours na kailangang mag-stay


yung patient sa PACU. As long as okay na siya, stable yung VS niya,
no other subjective complaints at nagagalaw na niya yung 4
niyang extremities, pwede na siyang itransout. Personally, kapag
GETA, I usually assess the patient after 1 hour then transout na
agad if walang problems. Tip, kapag GETA, para sure ka na wala
nang residual neuromuscular blockade yung patient, ask them to
lift their head off the bed for 5 seconds. If kaya nila, then ok na sila.
Kaya na nila isupport yung airway nila. Same test din ang gawin
ninyo for patients who were given post op opioids (usually
morphine)
• Kapag SAB naman, medyo mas matagal. Kailangan bumalik muna
yung sensory at motor function ng lower extremities ng patient
bago mo sila itransout.
• Kung ittransout niyo na yung patient, mag-entry kayo sa chart niya:

PACU TRANSOUT ORDERS/NOTES

Patient seen and examined.
Currently awake, conscious, coherent, responsive,
NICRD and appears to be comfortable.
➢ No subjective complaints
➢ Stable Vitals Signs (BP, HR, RR, SpO2, Temp)
➢ Good activity
➢ Adequate Pain Control
May transfer the patient back to ward

PACU-ROD/IOD

• Ipa-sign niyo na sa PACU-ROD.


• For patients na nasa Area 4 (intubated patients/critical care
patients)
o After receiving the patient, required tayong gumawa ng
Incoming notes for each patient admitted at Area 4
(Malinaw, AREA 4 lang. Yung iba, walang incoming notes).
o Hindi ninyo ipapasok sa chart yung incoming notes. Iccollect
niyo lang yun then ippass at the end of the 1 week Anesth
Rotation.
o Yung intern lang na tumanggap sa patient during his/her
duty ang gagawa ng incoming notes.
o Kapag patapos na yung duty mo at hindi pa nattransout
yung patient sa area 4, endorse mo yung patient sa next
IOD (informal endorsement lang naman, mga to-do’s, etc).
Yung intern naman na papalit sayo ay ang required na
gumawa ng Progress Notes for that patient.
o Essentially, gagawa at gagawa ng progress notes yung IOD
para sa lahat ng mga Area 4 patients as long as hindi pa
sila nattransout.
o Again, collect niyo lahat ng incoming notes and progress
notes. Staple niyo yung lahat ng notes for a certain patient
para hindi nakakalito.

B. DUTY PM
• Same as Duty AM
• 7PM to 7AM
C. LCB
• 7AM to 4PM
• Mondays, Tuesdays and Thursdays: All cases sa LCB Room 11
• Wednesdays and Fridays: All cases sa OB OR sa 4th floor
• Attendance: index card, pa-sign kayo sa Anesth after ng LCB Post
ninyo
• Usually mag-oobserve and help-out lang kayo sa Anesth resident
dun. Ask questions kung may mga tanong. Patulong/paturo mag
spinal anesthesia or maginduce ng patient for GETA.
D. OPD
• 7AM to 2 PM
• Recovery Room sa Major ORs ng OPD
• 2nd floor, OR Complex
• Attendance: may logbook dun sa Recovery Room
• Same lang din nang ginagawa sa PACU pero mas benign yung
patients.
E. POST
• Tulog/Gala unless may naka-schedule na lecture or SGD or activity.

II. ACTIVITIES
A. CONFERENCE
• Wednesdays, 2pm, Wear your coat
B. LECTURES
• Wala kaming lectures pero supposedly may 5.
• Tanungin niyo na lang si Maam Erna kung ano yung schedule ng
lectures
C. QUIZ
• Samplex
D. SGD
• Yung consultant yung pipili ng topic. Mga post op complications
• Better kung may actual patient. If hindi nakahanap, idiscuss niyo
na lang yung topic.
• Tanungin niyo na lang si Maam Erna para sa schedule nito.
