We are submitting herewith our Request for First & Partial Payment/Inspection covering from
the period of July 04, 2019 – October 14, 2019 in the amount of Eleven Million One hundred
Seventy Eight Thousand One Hundred Seventy Five pesos & 97/100 (Php 11,178,175.97) .
We have accomplished the work satisfactory in accordance to the term and condition of the
contract. We also certify that the Performance Security of Contractor’s Comprehensive All Risk
Insurance (CARI) is valid and premiums have been paid on time. Detailed Quantity Calculation,
supporting papers and pertaining documents are hereto attached for your review and approval.

We hope that the above is in order and request your preferential attention thereof.

That, the B. VICENCIO BUILDERS (BVB) INC.___has a contract with the Republic of the
Philippines under the Department of Public Woks & Highways(DPWH), Calbayog City, Samar

That the said Contract is for Contract ID : 19IJ-0121 002: Protect Lives And Properties Against
Major Floods, Flood Management Program Construction/Maintenance Of Flood Mitigation
Structures And Drainage System, Construction Of Flood Control Structures Along Calbayog River
(Upstream), From Jasminez Bridge To Payahan Bridge

That in compliance with Department Circular No. 17, Series of 1996, The B. VICENCIO
BUILDERS(BVB) INC. hereby states the gross receipts has been declared with the Bureau of
Internal Revenue for Income Tax Purposes;

That we have submitted our Monthly Certificate of Payment No.____1___for our construction
works for the period covering July 04, 2019 – October 14, 2019.

That we have not given any money or gift, nor made promises of anything to any persons,
employees, or officer of the Department of Public Works and Highways in connection to the said
project:

That B. VICENCIO BUILDERS(BVB) INC.___has fully paid all workers/laborers, subcontractor,

suppliers and all persons or parties who have done or furnished materials supplied, supplied
services, and other inputs on work accomplished on the project as of October 14, 2019 and such
payments were all in accordance with law;

That this affidavit is being executed to support our claim for the First Partial Payment for work
accomplished anent above said project and to comply with the legal requirements.
