Comments: *HIGH RISK***
- Signs of active roach infestation present. All stages of roach development found throughout the
meat department of the facility, in the unsealed grain storage area, and the non-refrigerated
produce shelving on glue boards. Proof of pest control services could not be furnished at time of
inspection.
- Live, active roaches found under the meat department cutting boards, butcher blocks, inside an
unused band saw, and under/inside the meat display coolers along the railings.
- Live and active roaches present inside the makeshift diving wall inside the meat department.
- Live and active roaches found on glue boards between shelving units containing raw produce,
such as coconuts and root vegetables.
- One live freshly molted German roach and other German roaches found between bags of
rice/grains on the grain aisle shelving.
- Butcher blocks at the end of the meat cutting/preparation area are split and have roaches
present.
- Roaches found under hanging soiled linen aprons in the meat department.
- Roach scat found covering multiple surfaces; cutting boards, the crack in the butcher blocks,
along the meat department false wall window, along the edges of the produce shelves, on the
rice and other grains storage shelves.

*LOW RISK*
- Multiple roach fragments found on the shelving units and bags of various grains.

Comment: Cardboard boxes being used to house raw produce product on the shelving.
Cardboard is contributing to the roach infestation. Remove.

Boards have deep grooves, gashes, and chunks missing. Remove/discard.

and with heavy mold infiltration on the surface of the block. Remove/discard.
