AFFIDAVIT OF ONE AND THE SAME PERSON

I, MARIENETH SIBUG SABIO, of legal age, Filipino citizen, single, and a

resident of Block 12 Lot 13 Southville 3A Extension San Pedro Laguna, after having been
sworn in accordance with law aver that;

1. My name in my birth certificate is MARIENETH SIBUG SABIO. Attached hereto

is a copy of my birth certificate as Annex “A”;

2. I have used the name MARIENETH SIBUG SABIO, in all of my records and
dealings such as government IDs. Attached hereto is a copy my Philhealth ID as
Annex “B”.

3. When I was arraigned on January 2019 before the Regional Trial Court of Manila
Branch 2, I was nervous and confused at that time and that my name in the court
records was MARIENETH PAULE SABIO.

4. The correct middle name is Sibug and not Paule. Paule is the middle name of my
mother.

5. In order that there will be no problem in my application for probation, I want to

correct my middle name in my application for probation.

6. I am executing this affidavit to attest that “MARIENETH SIBUG SABIO” and

“MARIENETH PAULE SABIO” as appearing in the copy of my certificate of Live
Birth and Philhealth ID are ONE AND THE SAME PERSON.

7. I am executing this affidavit to attest to the truthfulness and honesty of the

foregoing facts and for all intents and purposes it may serve.

1. That we know personally FRANKLIN MIRADOR MOSELINA a.k.a.

DANILO MIRADOR MOSELINA who was born on December 27, 1961,
Filipino Citizen, of legal age and single;

2. That we know him to be using as his name DANILO MIRADOR

MOSELINA in all his private and public dealings instead of FRANKLIN
MIRADOR MOSELINA her true and correct name appearing on her Birth
Certificate;

3. That DANILO MIRADOR MOSELINA and FRANKLIN MIRADOR

MOSELINA is one and the same person;
