INFORMATION

The undersigned Assistant City Prosecutor accuses Fernando Y. Fernandez of the crime
of violation f Sec. 5(a) of R.A. No. 9262 or the Anti-Violence against Women and their Children
Act of 2004, committed as follows:

That on January 20, 2020 in the City of Angeles, Philippines, and within the
jurisdiction of this Honorable Court, the said accused, with deliberate intent, did
then and there commit acts of physical abuse against one Heidi Perlas K.
Fernandez, by repeatedly punching and kicking her in different parts of her body
which caused her to sustain injuries that needs at least 30 days of medical
attention.
