1.0 PURPOSE
1.1 To gather preliminary valid information of the students both old and new.
1.3 To provide concise and factual behavior and progress of the student during his/her stay in
school.
2.0 SCOPE
2.1 This procedure covers the Admission and Anecdotal form of the Guidance Office.
2.2 This also covers the valid data and information that can be used to examine the status of the
student.

4.16 LRN # - a permanent twelve (12)- digit number which the pupil, student or learner
shall keep while completing the basic education program, regardless of transfer to
another school or learning center in the public or private sector, and promotion/moving
up to the secondary level.
