SECOND SUPPLEMENTAL CONTRACT OF LEASE

This Supplemental Lease Contract made and entered into by and between:

________________ ,of legal age, Filipino, residence and postal address at Barangay 4 an Simon, Magallanes
Village, Makati City, and____________, of legal age, Filipino, married to _____________, with residence and postal
address at 65 MagallaNes Avenue, Magallanes Village, Makati City, hereinafter known and referred to as “LESSOR”;

- and -

______________________., a domestic corporation duly organized and existing under and by virtues of the
laws of the Republic of the Philippines, with office address at 6750 Ayala Avenue, Makati City, represented herein
by its ____________________, and hereinafter referred to as the “LESSEE”.

RECITALS

WHEREAS:

The LESSORS and the LESSEE on executed a Lease Contract, covering the abovementioned LEASED PREMISES, which
contract is identified as Doc. No. 41, Page No. 24, Book No. XXV, Series of 2014, in the notarial register
__________________ for the LESSOR, and identified as Doc. No. 115, Page No. 24, Book No. I Series of 2014, in the
notarial register ___________________ for the LESSEE copy hereto is attached as “ANNEX A” and made an integral
part of this Supplemental Contract Lease.

The Lease Contract with a period of ten (10) years, and the commencement date shall be from April 1, 2013 to March
31, 2023, as agreed upon by BOTH parties.

It is the agreement of BOTH parties, that the indicated period of the Lease Contract be EXTENDED from TEN (10)
YEARS to SIXTEEN (16) YEARS, commencing from APRIL 1, 2023 to MARCH 31, 2029 as the latter reflects the true
intention of the parties regarding the period of effectivity of Lease Contract.

It is also the agreement of BOTH parties, that the escalation rate be increased to ten percent (10%) for the 5 th year
of the Agreement only. For the succeeding years until its expiration date shall maintain an escalation rate of five
(5%).

NOW, THERRFORE, for and in consideration of the above Recitals, the parties agree as follows:

1. Terms: To have and to hold for a term of SIXTEEN (16) YEARS, effective from APRIL 1, 2023 to MARCH 31,
2029. This lease, however, is subject to termination by LESSEE at any time upon thirty (30) days written
notice to LESSOR.

2. Rental: LESSEE agrees to pay the following rental for said premises.

It is hereby expressly understood that with the foregoing amendment, all other terms and conditions of
said Lease Contract not hereby expressly amended, shall remain in full force and effect and that the parties hereto
agree that this amendment made an integral part of such Lease Contract.
