Creditor Efren A. Duran Jr. that I am indebted to the latter in the amount of

to pay Efren A. Duran Jr. on or before _____________ ( ___ month/s) in the amount of
____________________ every payday ( every 9th or 24th of the month) until my debt is fully paid.

I have already paid the advance interest of six (6%) percent per month or _______________
pesos and deducted from the total loan.

I authorize the creditor’s possession of my LANDBANK ATM CARD until my debt is fully
paid. I further authorize creditor/Mr. Efren A. Duran Jr., his heirs, assigns or his legal
representative to withdraw the above mentioned amount of __________ every payday from my
Salary deposited at Landbank with the corresponding ATM Card (Payroll Account) with account
number ___________________________ and PIN __________________.

I acknowledge that I have no argument with the said debts, which were incurred by
myself, and for which I am solely responsible. I hereby undertake to deposit my ATM to the
creditor and desist from making any online bank transfers, request for the issuance of a new
ATM card during the existence of this loan.
