A BARANGAY RESOLUTION INTERPOSIN NO OBJECTION TO OPERATE WATER REFILLING

STATION OWNED BY MRS. O VELADO ERPELUA, LOCATED AT LOT NO. 1957-B-1-B,
BARANGAY LAWA-AN, ROXAS CITY

WHEREAS, the demand for refilling stations as an alternative source of safe drinking water in
the Philippines has increased such that water refilling stations have proliferated nationwide;

WHEREAS, the owner submitted the complete requirements for the operation of the refilling
stations such as:

a) Certificate of Occupancy issued by the Department of Public Works, Transportation and

Communications;
b) Certificate of Business name Registration issued by the Department of Trade and Industry,
Region 6; and
c) Health Certificate issued by the Department of Health, Roxas City;

WHEREAS, the registered business name is CBM Water Station, located at 3’R Sister Building
Mabasa Subdivision, Brgy. Lawa-an, Roxas City, Capiz;

WHEREAS, the owner/operator of the business are Sps. Rodolfo and O Erpelua as their as their
names appeared on the Provincial or City treasurers Real Property Tax Receipt;

NOW, THEREFORE, BE IT RESOLVED, as it is hereby resolved to issue a Brgy. Resolution

interposing NO OBJECTION TO OPERATE a water refilling station owned by Sps. Rodolfo and
O Erpelua, located at Brgy. Lawa-an, Roxas City;

RESOLVED FURTHER, that Mrs. O Erpelua will furnish the names of a least twenty household
who are supporting the operations and indeed provides advantage to its neighborhood;

RESOLVED FINALLY, that the above mentioned household will affix their signature to signify
their conformity.
