Documents

Remember, your documents


must:
Be original or certified by the issuing
agency.
Be unexpired, unless specifically
noted.
Be in English, or accompanied by a
certified English language
translation.
Not be mutilated or damaged (we
will confiscate any that appear
fraudulent).
Show the same name:
If you have a middle name or
suffix, at least one document
must show the full name.
If your name was legally
changed, you must have
original or certified copies
showing the change (for
example marriage certificate,
divorce decree, or court-issued
documents).
If your name has changed
multiple times, you must bring
multiple documents that link all
names.

Social Security Status

Passport - Foreign (non-United

New York State Residence

Card or Official Transcript - United
States (if issued within 1 year) (2
name points)

Additional Proof of Name


The documents you selected to meet the
other proof requirements also satisfy your
proof of name requirement (6 name
points), and you didn't select any
additional documents to bring.

Payment for Fees


Your fee will not be more than:

$136.00 for a Learner Permit (Class


DJ/D or MJ/M)
$98.50 for a License (Class D or M)

Be prepared to take a written


permit test
If all of the documents you bring are
acceptable and you are applying for a
permit, you must pass the written test
before the permit is issued. You can learn
more about getting a permit or study for
the test using our driver's manual practice
quizzes.

not required, and are not available for all DMV
locations. You can read more information within
