6-501.111 (C) - CONTROLLING PESTS
Observation: Excessive mouse droppings observed throughout the facility including: on the clean equipment rack
near the three compartment sink on the unused tomato dicer and unused party size pizza pans; on the base of the
dough mixer stand; on the food storage rack in the boxes holding the packets of seasonings; on the shelf below
the dough sheeter with the clean pizza pans; in and on the dough sheeter; and along the edges of the floor along
the walls throughout the facility. Mouse dropping and nest observed in the bathroom on the bottom shelf of the
chemical/cleaner storage rack. Live mouse found beneath the computer desk in the small office area of the
kitchen. Employees indicate that recent pest control maintenance was received but no documentation is available
for review. Per phone call with supervisor, staff indicated that sheeter must be cleaned by professional with the
proper tools.
Corrective Action(s): Discard all rolled dough that went through the sheeter. Discontinue use of the dough sheeter
until it has been professionally cleaned. Notify all shifts that sheeter cannot be used. Dough may be hand rolled
with rolling pin on a clean, sanitized counter or rolled dough can be provided from another licensed store until
sheeter is returned to use. Notify this Division about cleaning of dough sheeter. Clean and disinfect all equipment
and surfaces where mouse activity was observed. Provide documentation of active pest control maintenance by a
licensed pest control operator to eliminate mice from the facility. Correct By: 29-Jan-2020
Actions Taken: Staff discarded all rolled dough and discontinued use of sheeter. Staff decided to hand roll and
toss the pizza dough until sheeter can be returned to use. 1/28/20
Actions Taken: Per phone call with GM, she is personally disassembling and deep cleaning the dough sheeter
before store opens on 1/29. Licensed pest control company has been contacted and will be returning to inspect the
entire premises (including other areas of the building that are not part of store) and providing additional pest
treatment as necessary on 1/29. Copy of pest control documentation will be sent to this Division. Staff began deep
cleaning of facility to address all areas with pest contamination after inspection on 1/28 and will complete
cleaning before store opens on 1/29. A site visit will be completed to verify actions above later this week.

4-204.11 - VENTILATION HOOD SYSTEMS, DRIP PREVENTION
REPEAT OBSERVATION The vent hood filters on the pizza oven are incorrectly installed horizontally.
Corrective Action(s): Install the vent hood filters to the pizza oven vertically to allow for correct conveyance of
grease and condensation. Design the exhaust ventilation hood system to prevent grease or condensation from
draining or dripping onto food, equipment, utensils, linens, single service or single use articles. CORRECT
BEFORE THE NEXT INSPECTION.
Code citation: Exhaust ventilation hood systems in FOOD PREPARATION and WAREWASHING areas
including components such as hoods, fans, guards, and ducting shall be designed to prevent grease or
condensation from draining or dripping onto FOOD, EQUIPMENT, UTENSILS, LINENS, and SINGLE
SERVICE and SINGLE USE ARTICLES
6-501.11 - PHYSICAL FACILITIES - GOOD REPAIR
Observation: A. Several broken floor tiles noted including near the front kitchen entrance, next to the pizza oven,
and near the dough sheeter.
B. A broken ceiling tile noted near the side entrance to the kitchen above pizza dough and boxes.
Corrective Action(s): Repair or replace noted floor and ceiling tiles. Maintain the physical facilities so they are in
good repair at all times. CORRECT BEFORE THE NEXT INSPECTION.
