AFFIDAVIT OF ADMISSION OF PATERNITY

I, DANILO F. DEMAPE, of legal age, Filipino, single, and a resident of Barangay
Dayhagon, Municipality of Sigma, Province of Capiz, Philippines, after having been duly
sworn to in accordance with law, hereby depose and say:

1. That I am the natural and biological father of JENNY BABES TUSTADO who
was born on 12 July 1995 at Davao Regional Hospital, Apokon, Tagum City,
Davao del Norte Philippines;

2. That the said child was born out of wedlock and that the name of her biological
mother is BABYLIN ALISTE TUSTADO; and

3. That this Affidavit is executed to attest to the truth of the foregoing facts and to
inform the Office of the City Civil Registrar of Tagum City of my decision to
process the registration of this legal instrument for purposes of acknowledging
the paternity of my daughter, JENNY BABES TUSTADO.

IN WITNESS WHEREOF, I have hereunto set my hand this 24th day of January,
