GUIDELINES FOR PROVIDENT FUND SETTLEMENT

As per the PF Rules, withdrawal of PF accumulations is applicable only if :

1. The person is currently unemployed for more than 60 days

2. The person is working in an organisatioin which is exempted (not covered by PF)

3. The Person is going abroad post leaving services as a proof of which copy of VISA and Airticket
would be required.

If you meet any of the above conditions, kindly send your following two duly filled forms (Form
19 & Form-10C)

A. Form 19 :

1) Name of the member - (in capital letters)

2) Father's/Husbands Name - << Mention your Father's Name as mentioned during
