SUBJECT: LIST OF DEFECTS -(CIVIL WORKS) AS AT 26th NOVEMBER 2010.

As agreed in the Progress Review Meeting held on 22nd November 2010, we are herewith
forwarding the List of Defects in Civil Works of the Four Building Towers and Sewer
Treatment Plant of the above project after jointly examining the work .

Please note that the following representatives from the respective institutions checked the
civil work carried out by the contractor, Euroville Engineers and Constructors (Pvt) Limited
of Jaffna and agreed that the rectification of the defects which have been listed in the
annnex is a responsibilty of the contractor.

As the inspection team did not comprise members with the knowledge and experience
over the electrical and mechanical engineering fields, defects of electrical/mechanical
works in the Buildings or Sewer Treatment Plant were not notified. Therefore,latest
electrical /mechanical defect list issued by us could be consider asacceptable.

DEFECTS IN SEWER TREATMENT PLANT-(CIVIL WORKS)

1. It can be observed that Baffle plates, at the entrance in inclined position of Type A
and type B were not properly fixed as per the drawings.

2. Cover provided for the grit chamber with heavy concrete plates are not suitable as
daily cleaning is necessary.

3. Two oprating valves for alternating diversion of sludge in to drying beds.

Above three items could be identified in civil works. However,a proper inspection will be
done during the comissioning of plant and any defects in the plant will be notified later. It
will be carried out by mechanical and electrical engineers jointly.
DETECTS (CIVIL WORKS) IN PHASE 1 WORKS - TOWER B & TOWER C

1. Door locks provided to toilets and Bathrooms cannot be opened with the given keys.
After pressing the lock from the toilet inside doors can be closed and then from
outside cannot be open with the key. Under this condition Inspection Team is in the
view that the locks shall be replaced with proper ones.

2. Vertical cracks near the column fall of plaster need to be repared.

3. The sinage "EXIT" has not been fixed properly and seen hanging by a side.

4. Glass of the EXIT door is missing. Contractor has to supply and fix it.

5. In tower C, there is a water leakage inside the duct.

DETECTS IN PHASE 2 WORKS (CIVIL WORKS) - TOWER A & TOWER D

1. Bath Shower in Toilet A213 is not working.

2. Exhaust fan of the toilet block is not working.
3. In to the Balcony of Room A 514, water leaks from a downpipe.
4. In tower A, floor tiles are missing in some places near the entrance.
5. Water stagnating in the Balcony of Room A 504.
6. In the Bathroom at the Top floors, water flow through the shower is very slow and
need to be enhanced.It can be observerd that pipe lines connecting water tank and
showers at these floors are very long and and U tube shape. To avoid head losses
due to long length and water connections at down stairs, it is suggested to loop the
water supply line in both Tower A and Tower D.
