PROMISSORY NOTE

FOR VALUE RECEIVED, I and /or We all of legal age ,jointly and severally promise to pay_____________

of____________%per month ,payable within( 58)days from the date of this note,payable on daily installment
basis,without need of notice or demand.

In the case of non payment of the said sum or any daily installment due on this note,I/WE jointly,and
severally,agree to pay an additional sum equivalent to (______%)percent per month on the total amount due and
unpaid,as the penalty charge,from the date of default until fully paid. In the event the LENDER discovers any
defect,fraud or misrepresentation in obtaining the loan as evidenced by this note or when there is any defect or
misrepresentation in the collateral placed as security thereof or there is a violation of any of the terms of the credit
agreement or of this Promissory Note or default on any or single installment on due date,this note shall immediately
become due and payable even before the maturity date stated above.I/WE hereby authorize,permit and empower the
LENDER,at its sole option and without need of notice,to apply to the payment of this Note,in the event it is not paid at
maturity or in case of failure to pay any daily installment,whatever funds and other financial benefits with the
LENDER,Any partial payment on overdue installment and/or outstanding balance,as the case may accepted by
___________________________________(Co.)shall not,In any way,be considered as extending the time for the
payment of any installment or as a modification of the terms and condition hereof.

All correspondence relative to this Note,including demand letters,summons,or notification of any judicial or
extrajudicial actions shall be sent to the BORROWER or CO-MAKER at the address indicated below and the mere act of
sending any correspondence by mail or personal delivery to said address shall valid and effective notice to the Borrower
and CO-MAKER for all legal purposes,and that if it has been returned unclaimed to the LENDER,the same shall not relieve
the Borrower from the effect of such notice.It is further agreed that it upon such default,the service of an attorney or
that of a third person or agency are availed of,an additional sum equal to twenty five percent(25%)of the total
obligation due thereon,shall be paid to the LENDER or holder hereof,plus an additional sum for liquidated
damages,expenses of collection and legal costs.It is expressly agreed that all legal action arising out of this note shall
only be brought in or submitted to jurisdiction of the proper court in Tagbilaran City Bohol or in any municipality or city
where any branch of the holder is located,This Promissory Note is secured by a

I/WE hereby acknowledge that I/WE have carefully read and understood all the foregoing and I/WE affirm
that at the time,I/WE affixed my /our signature hereto,all the blank spaces have been correctly and completely filled-
up.However,should there be corrections or entry which needed to be made for whatever errors or blanks which need to
be filled-up either in this note,loan documents and other related mortgage documents,I/WE hereby voluntarily and fully
authorized the LENDER to make such corrections or to fill it up.
