AFFIDAVIT OF GUARDIANSHIP

I, WILFREDO MIRANDA Y ALON of legal age, resident of Sitio. Canipaan,

Brgy. Banquerohan, Cadiz City, province of Negros Occidental, after having sworn in
accordance with law, do hereby depose and state THAT:

1. That, I am the father of Jay-arr Sevilleno Miranda who together with his wife
Frezamae Ogaban Miranda, seek to adopt the child Christine Higtenta;

2. That, I am gainfully employed as a Boat Captain of KOALL Company;

3. That, I fully support the adoption plans of my son and daughter-in-law;

4. Thus, I manifest my willingness to act as guardian and assume the

responsibilities of the adopting parents to provide support for the minor
Christine Higtenta should anything untoward happen to them;

5. That, I am executing this affidavit freely and voluntarily, and completely

aware of the possible responsibilities I have assumed in relation to Christine’s
adoption;

6. I am executing this affidavit of guardianship to attest to the truth of the

foregoing.
