by all authorized parties?

approved by competent personnel?

7 Has the Pre-Job safety meeting been conducted prior commencing the work?

8 Have all of the changes on the isolation been recorded properly in the
isolation certificate?

9 Has the management of change procedure (MoC) been applied to any

deviation or changes of procedure and/or method of work?

1 Have all of the isolation equipment been applied/installed refer to the

isolation design?

2 Has the initial integrity testing been performed to all isolations?

3 Have all isolations been secured to prevent any unauthorized operation?

4 Has isolation integrity monitoring been performed at suitable intervals and

recorded as part of the shift handover?
