The testimony of the witness Mrs. ANDREA S. LIM is being
offered to prove the following:

1. That she is the mother of the petitioner;

2. That she knows the personal circumstances of the petitioner;
and
3. That the entries sought to be changed in the petitioner's
Certificate of Life Birth are erroneous.

Andrea, in the course of her testimony, will identify evidence

pertinent to this petition

I, ANDREA S. LIM, of legal age, Filipino, married, Lot 29

Ciudad Grance, La Trinidad, Benguet, after having been duly sworn
to in accordance with law, hereby state that:

I am answering the questions propounded by Atty. Janice Nay-

ud of MAXWELL LAW FIRM fully conscious that I am under oath and
that I may face criminal liability for false testimony or perjury;

This Judicial Affidavit is written in English, a language known

and understood by me, and the examination and execution thereof
were done at MAXWELL LAW FIRM, U902B UDonote Building, Baguio
City; and

This Judicial Affidavit shall serve as my direct testimony in this

case.

III. JUDICIAL AFFIDAVIT PROPER

Q1 : Please state your name, age, residence, and occupation.

A1 : ANDREA S. LIM, 56 years of age, a resident of Lot 29
Ciudad Grande, La Trinidad, Benguet

Q2 : Why are you here now?

A2 : To directly testify regarding the erroneous entries in the
Certificate of Live Birth of the petitioner

Q3 : Do you undertake to answer the questions to be asked of you,

fully conscious that you will do so under oath, and that you
may face criminal liability for false testimony or perjury?
A3 : Yes, attorney.
Q4 : How are you related to the private petitioner Andy Lim?
A4 : Yes. She is my daughter.

Q5 : Do you have any proof that the petitioner was indeed your
daughter?
A5 : Yes. I have here with me the Certificate of Live Birth of the
petitioner.
(Manifestation. May we pray the Honorable Court that the
Certificate of Live Birth issued by the PSA to Rachel S. Lim, and
was marked as Annex A in this Petition be adopted and marked
as Exhibit “A”)

Q6 : Being the mother of the petitioner, are you aware of any

problem regarding the fact of birth of the petitioner, Andy Lim,
if any?
A6 : Yes, attorney. I know that I late registered the fact of birth of
my daughter with the Local of Civil Registrar of La Trinidad,
Benguet however instead of putting the word Rachel, I made a
mistake by putting the name Andy. And instead of putting
1990, I made a mistake by reflecting 1920 as the birth date of
my daughter.

Q7 : After learning the mistake in the birth certificate of the

petitioner, what did you do, If any?
A7 : Because I was not aware at that time of the process on how
to correct the entries made in the live birth certificate of my
daughter, and considering the first registration was already a
late registration and can no longer be corrected under
administrative correction, I thought the best remedy to rectify
the error was to register the same with the Civil Registrar of La
Trinidad.

Q8 : Madam witness, do you have any proof that indeed your

daughter was born on the year 1990 and not on 1920?
A8 : Yes Ma’am. I have a copy of her baptismal certificate and
marriage contract showing the true and correct year of his birth
is 1920 and not 1990.

Marriage Contract issued issued by the PSA to Rachel S. Lim,
and was marked as Annex B in this Petition be adopted and
marked as Exhibit “B”)
