JOINT AFFIDAVIT OF
TWO DISINTERESTED PERSONS/ ONENESS OF NAME

We, JEBIE T. DIONISIO and ALMIE T. DE ASIS, both of legal ages, Filipinos, and
residents of Tondog, Tangalan, Aklan, having been duly sworn to in accordance with law, hereby
depose and state:

That we both personally know JHOAN T. LAMEDA, because we had been neighbors
for so long;

That in her Certificate of Baptism dated December 25, 1988 her name was mistakenly
indicated as “JOAN T. LAMEDA” instead of “JHOAN T. LAMEDA”;

That “JOAN T. LAMEDA” and “JHOAN T. LAMEDA” refer to one and the same
person, and that no other person or persons will be prejudiced by this declaration;

That we are executing this affidavit to explain the discrepancy between her first name as
reflected in her Certificate of Live Birth, and in compliance with her Confirmation
requirements;

That we are not related to him in any way, either by consanguinity or affinity;

As such, we execute this Joint Affidavit to attest to the truth of the foregoing facts and for
any other legal purpose that this Affidavit may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this 2nd day of August 2018,
