The article presents the technique of carrying out the unilateral spinal anesthesia with the use of
isobaric solution of ropivacaine. There is given a comparative analysis of hemodynamic parameters
at different stages of the surgery, depending on the method of anesthesia (classical spinal anesthesia
and monolateral spinal anesthesia). There was conducted the evaluation of the motor unit after
the surgical intervention. On the base of the obtained data, there are presented the conclusions
about the practicality and safety of various techniques of spinal anesthesia in this type of surgical
intervention.

ВВЕДЕНИЕ
Односторонняя спинальная анестезия становится все более по-
пулярной методикой при хирургических операциях, которые затрагива-
ют только одну сторону тела, особенно это касается ортопедической хи-
рургии нижней конечности. Наиболее важным преимуществом метода
ЦЕЛЬ ИССЛЕДОВАНИЯ
Провести сравнительный анализ влияния изобарического раствора
ропивакаина 0,5% на интраоперационную гемодинамику и выражен-
ность моторного блока, при выполнении спинальной анестезии тради-
ционным способом и методом односторонней спинальной анестезии.
МАТЕРИАЛЫ И МЕТОДЫ
В работе приняли участие 50 пациентов обоего пола в возрас-
те от 40 до 87 лет, которым проводилось бесцементное эндопротези-
рование тазобедренного сустава в условиях спинальной анестезии.
Показаниями к оперативному вмешательству являлись две патологии:
1) перелом шейки бедра, 2) коксартроз тазобедренного сустава. У зна-
чительного количества пациентов отмечалась сопутствующая карди-
альная патология в виде гипертонической болезни, ишемической бо-
лезни, обструктивной болезни легких и др. В зависимости от техники
выполнения анестезии (классическая или монолатеральная), пациенты
были разделены на две группы. Антропометрические характеристики
участников исследования представлены в табл. 1.
Всем пациентам, поступающим в операционную, проводилась ка-
тетеризация кубитальной вены, фиксировались исходные параметры
гемодинамики, проводилась предоперационная инфузия S.NaCl 0,9%, в
объеме 200–400 ml.

РЕЗУЛЬТАТЫ И ОБСУЖДЕНИЕ
Длительность оперативного вмешательства в первой группе соста-
вила 62,2±6,8 минуты, во второй группе 55±4,8 минуты. При этом время
нахождения в операционной у пациентов второй группы было меньше
(87,4±5,3) минуты, чем у пациентов первой группы (90,5±13,7 минуты).
При оценке гемодинамических параметров во время течения анесте-
зии отмечалось, что у пациентов второй группы наблюдались чуть бо-
лее низкие показатели АДс, АДд и АДср. и ЧСС, однако статистически
значимых различий не отмечалось (табл. 3).
При анализе гемодинамических показателей следует отметить, что
в первой группе с целью коррекции гипотонии в 5 случаях использо-
вались вазопрессоры, в отличие от второй группы, где вазопрессо-
ры не использовались совсем. Также следует указать на различия в

ВЫВОДЫ
1. Вышеописанная методика монолатеральной спинальной анестезии
достоверно позволяет добиться анестезии в требуемой конечности
без развития выраженного моторного блока в противоположной
конечности.
2. Гемодинамические различия в исследуемых группах были незначи-
тельными, однако во второй группе не применялись вазопрессоры,
и требовался меньший объем инфузии в сравнении с контрольной
группой, что свидетельствует о более стабильных гемодинамиче-
ских показателях.
3. Учитывая вышесказанное, предложенная техника унилатеральной
спинальной анестезии может быть методом выбора при проведе-
нии эндопротезирования тазобедренного сустава у пациентов с се-
рьезной сопутствующей патологией.
