DISCHARGE PLAN

MEDICATION:
 Patient and significant others were given instructions regarding the importance of continuing
medications as ordered by the doctor for total recovery, and were advised not to skip medications.
 Proper drug dosages and time of intake were explained to the patient as much as possible to facilitate
compliance with drug regimen.

EXERCISE/ENVIRONMENT:
 Teach the patient to do active range of motion with slow progression in frequency and provide for
assistance if needed.
 Promote rest and deep breathing exercise.
 Encourage significant others and family to provide a peaceful environment conducive for fast
recovery and healthy living and to provide a well-ventilated environment.
TREATMENT
 Discussed to the patient the complication of the condition to have more realistic expectations about
life expectancy.

HEALTH TEACHING
Health teachings were given with emphasis on:
 The importance of proper preparation of his enteral feeding to avoid contamination.
 The proper care of his jejunostomy feeding tube to avoid infection.
 Proper oral care to prevent further tooth decay and halitosis.

OUTPATIENT
 Encouraged patient and family members to have followed up check-up as ordered by the physician to
evaluate any complications or changes in his cancer and identify any further treatment to be done.

DIET
 Diet is in enteral or osterized feeding

SPIRITUAL
 Patient was encouraged to strengthen his faith in God, and become, emotionally and psychologically
healthy by praying together with his family every day.
EVALUATION, RESULTS AND RECOMMENDATIONS
On our last visit to our patient in the hospital on February 21, 2020, the patient went home against
medical advice since February 20, 2020. But we were able to communicate through his relative via phone.
According to his relative, he went on HAMA(Home Against Medical Advice) for the reason that his
chemotherapy schedule are always postpone because his WBC results are always low and that he is tired of
waiting and always complaining of being hungry. At home, he eats and drinks via oral and does not use his
jejunostomy feeding tube. He can consume half share of soft diet. His diet includes porridge, fruits and
vegetables. Consumed decoction of guyabano leaves, guava leaves, sugar-apple(atis) leaves every day.
Patient is not compliant on medication regimen. According to his relative, he did not drink his medication
and only relies of herbal medicines.
