Материалы и методы
Шестнадцать поросят, весящих между 10 и 30 килограммами, были
анестезированы Нембуталом (пентобарбитал натрия) с 25 миллиграммами
на килограмм , введенный внутрибрюшным образом после суточного
воздержания от еды и проветривания помещения, используя механический
вентилятор (Мэнли, Blease Limited). Дополнительные дозы Нембутала были
введены внутривенно, как требуется. С использованием матрасов
внутренняя температура поросят сохранялась в 37 градусах Цельсия . Правая
внешняя яремная вена была канюлирована с катетером Лебедя-Ganz для
измерения сердечного выброса, используя метод терморастворения (модель
9510, Лаборатории Эдвардса) и для смешанных венозных образцов крови.
Левый желудочек сердца был канюлирован через правую сонную артерию
для инъекции радиоактивных микросфер. Арбитражные образцы, во время
инъекций микросферы, были собраны аспиратором (Harward Apparatus
Company Incorporated) от катетера, вставленного в аорту нижней части
брюшной полости через левую бедренную артерию. Правая бедренная
артерия была канюлирована для прямого кровяного давления,
контролируемое с датчиком давления (модель CE 4S,

The well established circularity changes during acute pancreatitis are usually
attributed to hypovolemia which develops due tо massive plasma loss into the
tissues. However, despite aggressive fluid therapy, many of these patients die
which suggests that hypovolemia alone is an insufficient explanation for the
circularity collapse seen in patients who have severe pancreatitis.
Recently, there have been several reports of hemodynamic and vascular changes
in acute pancreatitis (1-7). Circulating toxic agents originating from an inflamed
pancreas are incriminated for these complications(7-13). Also, myocardial
depression by a peptide “myocardial depressant factor” (MDF) has been
suggested as a possible additional factor in causing failure of the circulatory
system (6, 7). However, this has not been unanimously confirmed (1,4).
Furthemore, it has been proposed (1) that cardiovascular and pulmonary
abnormalities are best explicable in terms of maldistribution of the peripheral
microcirculation from uneven vasoconstriction that leads to microcirculatory
shunting and limited tissue oxygenation for increased oxygen requirements.
Damage of the pancreatic blood vessels during acute pancreatitis is thought to be
responsible for the progression of the disease into a hemorrhagic form (5, 14-16).
Many therapeutic agents have been used in the treatment of acute pancreatitis
(17-20). The results of several investigations with dextran 40 have been promising
and its use is advocated (5, 21-23). However , some controversy still exists (24).
This study was undertaken to elucidate the hemodynamic and microcirculatory
changes in various visceral organs during experimental acute hemorrhagic
pancreatitis. The main emphasis was focused on evaluating the role of
hypovolemia in the development of microcirculatory changes and to determine
whether or not these changes can be prevented by adequate fluid and volume
substitution.
