Introduction
Manipal Hospital Dwarka Is a 380 bedded Super Specialty and Quaternary care
Hospital. The first in the country certified for ISO 9001 2000 for Clinical Protocols.
Nursing Care and Support Services. This institution has been accorded the National
Accreditation Board for Hospitals and Healthcare Providers (NABH) certification for
Patient care, National Accreditation Board for Lab Services (NABL), NABH Standards

Patient Safety
Ensuring patient safety is the responsibility of all staff in the hospital.

o Patient name and Hospital Number shall be used as unique patient identifiers.
Identification band shall be available with all in patients.
o Eliminate wrong site, wrong procedure and wrong patient surgery by using proper
patient Identification.
o Communicate to all the patients and their families on how to report concerns
about safety, and encourage them to do so.
o All medication orders to be legible and signed.
Verify medication order using the twelve R’s:
o Right client Right motivation/approach
o Right Dose Right of the client to refuse
o Right Route Right of the client to know the reason for the drug
o Right Drug Right evaluation
o Right time Right documentation

o All medications shall be appropriately labeled.
o Hand washing is the single most important procedure for preventing

transmission of infections. Hand washing is recommended before and after
patient contact, after touching soiled linen, wastes, immediately after removing
gloves.
o All incidents shall be reported.
o The Staff must take necessary safety precautions before, after and during
transport of patients to various areas
o Never leave a patient unattended

Patient Confidentiality and Privacy
➢ Do not discuss about a patient with anyone except the patient or his / her
representative
➢ Do not discuss about: a patient in the lift, corridors or any other public areas.
➢ The patient’s health record shall never be left unattended. It must be kept in
safe custody at all times.
Incident Reporting
➢ All incidents in any event or Circumstance resulting from your job, which
could have or has led to unintended or unnecessary harm to a person or a
compliant Loss or damage.
➢ The staff encountering the incident shall raise the report in the summit login

Sentinel Event
Sentinel Event is defined as any unanticipated event in the hospital resulting
in death or serious physical or psychological Injury to a person or persons and is not
related to the natural course of the patents illness. Sentinel events specifically
include loss of a limb or gross motor function and any event for which recurrence
would carry a risk of serious adverse outcome

NEEDLE STICK INJURY

If you sustain a needle stick injury. Take the following actions immediately:

• Wash the needle prick area with soap and water.
• Report it to your Nursing supervisor on duty and infection control Nurse and give
all details of exposure and source and your own immunization status. If the
source HIV/HCV and HBsAg status is unknown then send source blood samples
for estimation of HIV / HCV / HBV.
Exposed Health Care worker (HCW) also submits the blood for HCV and HIV
testing. Check the ant: HBsAg anti body if not tested before.
VACCINATION
Hepatitis B vaccination is recommended for all the health care workers The
hepatitis B vaccine is an injection that is generally given three-dose series on a O, 1,
and 6-month schedule.
Inj. Tetanus toxoid vaccine is recommended for HCWs and the dose is
repeated every 5 years
