1. Introductory Paragraph:
____ is a 5.2 year old who enjoys jokes, running around, playing and having conversations with
people. She is someone who needs to work on getting in good routine habits, as well as listening
to directions. If the task is fun, and energetic she will have no problem completing it, and will
listen to the directions being told. ____ is a very well behaved young girl who needs motivation
though. In order to have her perform tasks, many of the times she needs to be rewarded.
Designing the environment in a creative, fun setting is the way to get ____ to perform tasks
immediately. She is extremely joyful and cheerful, but prefers to converse with adults rather
than students her own age. She is someone who is always so eager to get started, run around and
go through the motions of each task. Throughout this semester, ___ will be working on fine
motor skills, and increasing her tolerance for motor skills within her capabilities. ___ does not
like loud noises or sensory items, therefore, creating an environment that avoids those will allow
her to have success with her goals.

2. Present Level of Performance:


___ is a wonderful young girl who is so enthusiastic and so eager to get to the tasks. After
conducting the assessment, I noticed her strengths were kicking, dribbling, catching, and
underhand rolling. ___ is always listening to explanations at the stations, and always
understands the rules and directions of the task being performed. She is so respectful of
the equipment I have provided for her as well as so willing to try out new things. Based
upon this, my goal for her is to try new equipment and challenge herself with tasks I have
provided. She is outstandingly intelligent and I truly believe she has it in her to test her
limits above and beyond. With the help of creative, fun tasks and rewards, my intentions
are to have her push herself to the next level and create that challenge for herself. She
enjoys being so successful and getting rewarded for it, therefore, by getting in a good
routine this semester, it will allow her to use these skills in the future. ___ is always
determined to kick, dribble, catch and roll the equipment I have for her in all different
tasks with great form!

Cognitive: ___ is always listening to explanations and understands the rules and
directions for each activity.

Affective: ___ is always so respectful and eager to help clean up and set up the next
activity.

Psychomotor: ___ is always so eager to kick a ball with great form and enthusiasm.
*Fitness: ___ does a great job with proper jumping jacks.

3. Long Term Goals and Short Term Objectives:


After conducting the assessment it is important for her to learn the proper form of
running, leaping, galloping and overhand throwing. This will be done by breaking these
long term goals down into short term goals. It is important for her to be able to perform
these skills properly, because when she is older she will be able to participate on teams
where these skills are necessary. For the cognitive aspect, ___ will be able to recognize
two of the skills, however, then later on be able to recognize and perform five out of the
ten locomotor skills. ___ is a young girl who struggles talking to her classmates and
peers, therefore, she will be working on saying hello to three students in her class and
then eventually being able to say hello to all students in the class. In relation to fitness,
___ was able to do jumping jacks for 30 seconds, therefore the short term goal is to get
her heart pumping and have her do them for one minute. And then eventually, have her
be able to do them for 2 minutes.

A. Cognitive Goal: ___ will be able to recognize and say locomotor/object


control skills being performed

1. Short term objective: ___ will be able to recognize and say the
overhand throwing and leaping skill being performed

2. Short term objective: ___ will be able to recognize and say 4 out of
the 10 locomotor/object control skills being performed

B. Affective Goal: ___ will be able to say hi to all the students in her class and
have a conversation with two of them

1. Short term objective: ___ will say hello to 3 students in her class.

2. Short term objective: ___ will be able to say hi to 5 students in her


class

C. Psychomotor Goal: ___ will demonstrate overhand throwing, leaping,


running and galloping with proper form

1. Short term objective: ___ will demonstrate overhand throwing and


leaping with proper form by participating in activities that include those to
help reinforce the skill.

2. Long term objective: ___ will demonstrate overhand throwing, leaping


and running with proper form that include those to help reinforce the skill.
D. Fitness
1. Short term objective: ___ will be able to do jumping jacks for one
minute

2. Long term objective: ___ will be able to do jumping jacks for two
minutes

4. Health Considerations
I do not believe there to be any health concerns.

5. Behavioral Considerations
___ is a young girl who does not like to get in the habit of a routine, therefore, with the
use of rewards it allows her to work on good behavior as well as routinely efforts.

6. Teaching Strategies

Practice: With the help of the practice teaching style, this will allow ___ to be constantly
perform the task over and over again. And because of that constant practice, she will start
to begin muscle memory and recognize and remember how a skill should properly be
performed.

Reciprocal: With the help of this teaching style, ___ will see me perform the task with
proper form and have to copy what I had done. By her visually seeing the task being
done, it will allow for a better understanding of the criteria and proper form.

Inclusive: With the help of this teaching style, ___ will be in a learning environment that
meets the needs of all kinds of abilities. Therefore, this environment will allow for
modifications and adaptations necessary for ___ to be successful.
7. Evaluation Plan
By conducting the assessment in the first lab session, I will use the data to work on
specific skills that lacked criteria throughout this semester. The use of fun, creative
activities will allow ___ to develop these skills needed as well as enjoying it in action. On
the last day, I will conduct a post assessment and with the use of creative, enjoyable tasks
have her willingly perform these tasks that she has been working on all semester and see
improvements. If data numbers increase, studies will have shown that improvements
were in fact made.

8. Services to be Provided:
Physical education instruction once each week at SUNY Cortland Adapted Physical
Education Lab for one semester (2/19/19 to 4/29/19).

9. APE Placement Decision Model


Due to ___ low locomotor/object control skill scores, she falls under the standard score,
however, I would recommend her for general physical education services. I strongly
believe that with her great listening skills and motivation to excel at new tasks general PE
would be sufficient for her. She would be in a general PE class, however, also receive
extra help to allow her to be successful. ___ is someone who is very mature and would
rather speak to adults than peers, therefore, a general PE setting would allow her to be
more successful communicating with peers her own age. I strongly believe that a five
year old’s score should be slightly higher, however, general PE would be a better learning
environment and allow her to get caught up to speed with the right teacher. ___ is a great
listener and understands, she just needs the extra time and help to work on these
necessary skills.
