CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

The contract made and entered by and between:

The *BUSINESS ESTABLISHMENT HERE owned and operated by CHINNIE GO, herein
after referred to as the "First Party".

-and-

EMPLOYEE NAME, of legal age, Filipino, with residence at ADDRESS, hereafter referred
as the "Second Party".

- WITNESSETH –

1. That the First Party is in need of the services of the Second Party for the operation of
BUSINESS ESTABLISHMENT;

2. That the Second Party has signified her intention, to which the First Party has accepted,
to provide the services needed by the latter;

3. That the Second Party hereby possesses the education, experience, and skill required to
perform the job as described therein;

4. That in the view hereof, the Second Party is hereby contracted as Masseur for the
period of specify date of start of validity of contract to its expiration, in consideration
of service charge of indicate percentage of the total gross and indicate percentage
commission and to be paid every 15th and 30th of the month;

5. That as a Masseur, the Second Party is expected to perform the following functions:
a. Attends to the massage services for the customers and provides utmost
customer satisfaction by anticipating to every guest's need with flexibility and
attention to detail.
b. Performs massage service procedure established by the management.
c. Thoroughly understands and adheres to proper massage treatment, customer
safety procedures, and management policies.
d. Be familiar with other services of the spa and promotions for the purpose of
assisting the customers and giving them the correct information.
e. Uses suggestive selling techniques to sell and promote services of the spa.
f. Shall report indicate number of hours a day or indicate number of hours per
week.
g. Does other related works assigned by the immediate superior.

6. That it is understood that this contract does not create an employer-employee

relationship between the First Party and the Second Party;

7. That this contract shall not be valid and binding as stated above and renewable upon
agreement of both parties; Provided, that this may be terminated by the SECOND PARTY
before the end of such period by giving notice thereof to the FIRST PARTY at least thirty
(30) days before the intended termination date; Provided, finally, that the FIRST PARTY
may terminate at any time the services of the SECOND PARTY for cause.
