COMPLAINT
WHEREFORE, it is respectfully prayed that, after due hearing, judgment be rendered ordering
the defendant to pay the plaintiff the amount of P1,000,000.00, with interest thereon at the rate of 12%
annum from January 2, 2017 until fully paid, plus the amount of P75,000.00 plus P3,000.00 per court
appearance, as attorney’s fees/
Plaintiff prays for such other and further relief as may be just or equitable under the premises.

COMPLAINT
1. Plaintiff is of legal age and a resident of No. 7, West Aguila St., Green Cross Subdivision, Quezon
City, while defendant is of legal age and a resident of No. 9, West Aguila St., Green Cross
Subdivision, Quezon City, where he may be served with summons;
2. Plaintiff is the owner of a house and lot locate at No. 9, West Aguila St., Green Cross Subdivision,
Quezon City;
3. On May 1, 2016, defendant leased the said house and lot from the plaintiff for a period of two
(2) years starting on the said date, at a monthly rental of P50,000.00 a month, payable within
the first five (5) days of each month;
4. By virtue of the said lease, possession of the said house and lot was delivered to the defendant
starting May 6, 2016;
5. However, defendant defaulted in the payment of the monthly rentals for six (6) months, from
January 1, 2017 to June 30, 2017;
6. Due to the default of the defendant in the payment of his rent, the plaintiff, through
undersigned counsel, sent him a letter dated July 10, 2017, terminating his lease and demanding
that he vacate the leased premises and pay his rentals in arrears within five (5) days from
receipt of the said letter; a copy of the said letter is hereto attached as Annex “A” hereof;
7. Defendant received the said letter on July 15, 2017, as shown by his signature at the bottom of
Annex “A” hereof, but he failed and refused, and until the present continues to fail and refuse,
without justifiable cause, to vacate the premises and pay his rentals in arrears;
8. Due to the refusal and/or failure of the defendant to comply with the plaintiff’s plainly just and
valid claim, plaintiff was compelled to file this complaint, and to engage the services of legal
counsel for a fee of P50,000.00, for which defendant should be held liable;
Plaintiff prays for such other and further reliefs as may be just and equitable under the
premises.

PETITION
1. That petitioner is of legal age, Filipino citizen, is the wife of Major R. Solar, residing at 177
Mayon, Quezon City; that respondent is the incumbent Superintendent of the Philippine
National Police, Quezon City with office at City Hall, Quezon City, where he may be served with
summons and other court processes;
2. That on August 30, 2017 Major R. Solar of the PNP, Quezon City while holding office was
arrested by superior police authorities for alleged participation in a robbery with homicide in
Quezon City;
3. That since then Major R. Solar was detained at the Police Sub-Station 5, Quezon City, without
any formal charge filed against him;
4. That Major R. Solar had not participated in the alleged robbery with homicide hence his arrest
and detention is without any lawful cause;
5. That Major R. Solar is restrained of his liberty without due process of law.
1. Order respondent and/or his agents to appear before this Honorable Court and produce
Major R. Solar and forthwith explain why he should not be released from detention
immediately;
2. Declare his arrest and detention as invalid and unconstitutional;.

COMPLAINT
1. Both the plaintiff and the defendant are of legal and residents of 3 Tayuman St., Tondo, Manila
and 4 Tayuman St., Tondo, Manila respectively;
2. Plaintiff is the owner of a certain personal property, namely, Hyundai Starex valued at
P800,000.00 and more particularly described as follows:
A motor vehicle namely a 4-cylinder 2009 Hyundai Starex Van that plaintiff rented to
defendant on December 5, 2017 to be used in plaintiff’s company outing, which van defendant
failed to return on the same day agreed upon by both parties.
3. The said property has not been distrained or taken for a tax assessment or a fine pursuant to
law, or seized under a writ of execution or preliminary attachment, or otherwise placed under
custodial aegis, or if so seized, that is exempt from such seizure or custody;
4. The plaintiff has demanded of the defendant the delivery of said property but the latter refused
and continues to refuse to do so; and
5. Plaintiff hereby executed a performance bond in double the value of the subject property, in
favor of the defendant, for the return of the property to the latter if such return be adjudged,
and for the payment of such sum as the defendant may recover in the action.

will, and hereby consents to act as such executor;

3. Petitioner was born on February 14, 1990, and her father’s name is James Bastos while his
mother’s name is Claire Santos. Her birth certificate showed that the name given was Baby Girl
Bastos, true copy of which is attached hereto as Annex “A.” Her school records until he finished
college carried the name of Baby Girl Bastos;
4. Petitioner’s present name is Baby Girl Bastos;
5. The family name, which carries, “Bastos,” means in English, perverted, uneducated or ill-
mannered” which evoked derisive laughter among her schoolmates and neighbours and has
caused him embarrassment in his school and social dealings;
6. The petitioner request that her present name be changed from Baby Girl Bastos to Sofia Santos,
which is the surname of her mother, to avoid derisive laughter among her schoolmates and
neighbours and to prevent embarrassment as she now intends to find work after college.
WHEREFORE, it is respectfully prayed that after due notice and publication in accordance with
the Rules of Court, and hearing this Honorable Court adjudge that the petitioner’s name of Baby Girl
Bastos be changed to Sofia Santos.

This is a verified petition filed by the petitioner, through her counsel, praying from
this Honorable Court to declare as null and void the duplicate owner’s copy of TCT
No. 267198, which had been lost and now considered beyond recovery and to order
and direct the Register of Deeds, Lingayen, Pangasinan to issue in lieu of the lost
one, a new duplicate owner’s copy thereof.
Let the petition be set for hearing on October 19, 2012 at 8:30 o’clock in the
morning on which date, time and place, the petitioner shall appear and prove his
petition, and that all persons having any opposition thereto may appear and show
cause, if any, why the petition should not be granted.
Let this order be published at the expense of the petitioner once in a newspaper of
general circulation in the province of Pangasinan, including the cities of Dagupan,
San Carlos, Alaminos and Urdaneta, and be posted by the petitioner in the bulletin
boards of the Registry of Deeds, Lingayen, Pangasinan, Sangguniang Bayan,
Mangaldan, Pangasinan, Sangguniang Panlalawigan, Lingayen, Pangasinan and at
the barangay hall where the property is situated.
