THE VOLUNTARY DEATH AID FUND (VDAF)
The Beneficiary Death Aid Fund (BDAF) is designed to mechanize the association to
help the members of San Mateo North District Teachers Association (SMNDTA) to ease their
financial and emotional burden during the loss of their love ones.
RULES AND REGULATIONS
That upon death of a beneficiary of a regular member (with three years as a
member) of the SMNDTA, the Paying member will receive a cash benefit of Php 15,000.00 as
approved unanimously by the body with One Hundred Three (103) votes on April 9, 2011
(optional for retirees) or Php 6,000.00 for Plan 6,000.00 for retirees.
A. Qualified Beneficiaries
1. Those whose names are listed in the Biodata Form of SMNDTA member.
2. Regular member can have ten (10) beneficiaries within 3rd degree of
consanguinity and 2nd degree of affinity but only entitled to three (3) death
occurrences.
3. SMNDTA Couple/Married members who opted to choose the same beneficiaries
shall be allowed to receive both their benefits provided they have updated
payments/contributions.
4. Beneficiaries of members should not be duplicated by other members. In case,
the nearest consanguinity member shall be entitled.
B. Fees and Dues/Corresponding Benefits
1. Plan 15,000.00
i. A member shall pay the amount of Two Hundred Pesos (P200.00) as seed
money for five (5) consecutive years.
ii. Pay One Hundred Pesos (P100.00) for every death of beneficiary among
members. A member will be benefited with the amount of P15,000.00 upon
the death of beneficiary for a member with 3 years membership and will stop
paying of 100.00 at the age of 70.
iii. This plan is optional to retirees who are still willing to pay the Beneficiary
Death Aid Fund.
2. Plan 6,000.00
i. Retiree member shall pay the amount of Four Hundred Pesos (P400.00) per
school year and be benefitted of Six Thousand Pesos (P6,000.00) upon death
of a beneficiary.
i. Retirees
P 400.00 per year for retirees who choose to have the Plan 6,000.
ii. In-Service
P 200.00 for five (5) consecutive years shall be paid upon renewal of
membership and P 100.00 thereafter for every death of beneficiary who is
paying for Plan 15,000
iii. Effectivity
1. Benefits or claims for BDAF shall take effect after 3 years membership.
Claims shall be received by the member after five (5) days upon death of
the beneficiary.
2. Two (2) lapses in paying dues shall disqualify the SMNDTA member to
receive benefits with evidence of notice.
iv. Notice of Proof of Death of a qualified beneficiary should be given by the
concerned member to the SMNDTA Office through the School Heads or to
the member of the BOD upon proof of decease.
v. Other Rules and Regulations
1. A retiree who chooses to stop paying dues to VDAF shall not benefit from
any of the plan regardless of how long he/she had been contributing to
the VDAF.
2. A retiree or an active member who failed to pay his/her dues shall be
reinstated upon payment of arrears not more than three (3) years.
a. Change/addition of beneficiaries shall be approved by SMNDTA
Board upon verification of status of membership along with proper
documents (birth certificate, marriage contract, adoption paper,
etc.).
vi. Special Provision
1. A member-retiree aged 70 who has not been served since his/her active
membership to age 70 years old may claim P 15,000.00 or P 6,000.00
only for his/her medical and personal consumption but all his
beneficiaries for BDAF are waived and thereafter, his/her membership to
BDAF is terminated.
2. A member who died at the age of 65 and who has not been served with 3
beneficiaries shall be entitled to avail P 15,000.00 or P 6,000.00 only.
3. The claim for the 6,000.00 shall be taken from the BDAF savings account.
4. The claim for the 15,000.00 shall be collected from all the members with
15K plan.
5. The claim shall be taken from the VDAF savings.
