I. Project Contacts:

To realize this goal, speakers from the City Health Office (Nurses,
Doctors, Barangay Nutrition Scholars, Health Welfare Personnel) will be invited
to conduct symposium and health awareness campaign in the school.
Distribution of hygiene and first aid kits, posting of information and advocacy
materials in strategic places, renovation of the school canteen, supplemental
feeding to undernourished students, repairing our water facilities for adequate
water, delivery of health services such as nutritional supplementation,
providing creative activities and recreational experience (Zumba, games, sports,
board games), establishment of menstruation area for students, and improving
the school clinic are the activities to be undertaken to make this project become
successful.
Project Phase will start on February 2019 and ends on May 31, 2019 (4
months) and will be implemented at San Rafael National and Vocational High
School. The project cost is______________.

3. Personal hygiene care and learning plan – step-by-step processes
of hand hygiene, menstruation management etc.
4. First aid kit/medicine cabinet – necessary to treat minor ailments
and injuries that happen in school and to help reduce the risk of
infection or the severity of the injury.

program. In this regard, the team may seek assistance from the stakeholders
specifically parents.

4. Distribution of hygiene
kit
The financial needs of the school for the implementation of the project will come
from donations of the stakeholders.

1. Monitoring of Team Performance – we need to monitor how
everything is going, including what the team is working on, which team
members are stuck on a task or what other tasks need to be done to move
forward with the project.
2. Regular status and progress report – it is important to know how
much more is needed to complete the entire project so we can identify existing
issues and make timely adjustments to get things back on track.
3. Providing recommendations and suggestions – we have to estimate
the percentage of project success or failure and provide valuable advice on how
to get the team to work together to prevent missing the deadline.
4. Ensuring that recommended actions are implemented – it is
important that the team is clear if there are any changes on the project plan.
We can remind the project team of any hanging tasks and issues that need
quick resolutions so we can drive the team to the direction it needs to go.
