Scenario Assignment 2

Provide Care for the Client Who Is Nauseated

Mrs. Janice Hatch is 86 years of age and resides at Evermount Retirement Home. Her diagnosis
includes type 2 diabetes and arthritis. She has just returned from the hospital following
gallbladder surgery. Her daughter arrives and is very concerned.
She is in her pyjamas lying in low Fowler’s position, 4 x 4 dressing on left upper abdomen. She
is holding her arm across her upper abdomen, saying that she is very nauseated, feels her
abdomen is distended and her mouth is dry, feels dizzy, and needs someone to help her.

1. Discuss the client’s verbal and nonverbal cues in order to gather information.
Verbal cues are: Mrs. Hatch says she is very nauseated, feels her abdomen is distended
and her mouth is dry, and feels dizzy.
Nonverbal cues are: Mrs. Hatch is in her pyjamas lying in low Fowler’s position, she is holding
her arm across her upper abdomen.

2. How will you interact appropriately with witnesses and family members in order to elicit
pertinent information?
By having a sensitivity and good interviewing skills being, considerate, empathic, and
decisive. Asking to the daughter how Mrs. Hatch has been coping with the activities of daily
living, including the psychological stress of her medical conditions; asking about concerns. Also,
as a Support Worker I would make observations and relate information to the team members,
provide feedback and document any change in client condition. I will ask Mrs. Hatch to point to
the area of pain, how long is the pain least, is it sharp? I use a chart to record signs and
symptoms of any illness, the care and treatment given to Mrs. Hatch. Some of the charts are:
SOAP, ADPIE, and DAR.

3. Implement appropriate safety measures that will ensure the client’s safety and well-being.
Some appropriate safety measures for Mrs. Hatch are: I would provide Mrs. Hatch with
non-skid socks for the prevention of falling and also 1/2 person assist because she is feeling
dizzy she would need someone to help her to walk around. I would make an adjustment to her
nutrition plan because she is diabetic, I would also check her blood sugar as well (if I am allowed

to do so) because she probably might be in hypoglycemia. She would also need to have her
dressing changed to prevent infection (since she is at a risk for infection), Mrs. Hatch would also
need assistance when showering and would need bars in the shower at the retirement home as
well a shower seat since she just had surgery. I would educate and assist Mrs. Hatch about oral
hygiene and introduce cold water, ice chips, ginger products, and room temperature broth or
bouillon if tolerated and appropriate to her diet. I would position her in an upright position when
eating all in helps of assisting with nausea. I would also practice patient centered care and
communication to get Mrs. Hatch involved so providing her with the necessary information she
would need in hopes of feeling better is also important. This will help her to understand her
current medical condition and even help her if she has any questions. I would also make sure that
her call bell is beside her, so she can use it when she needs assistance.

4. Apply problem solving skills to collect relevant data about the client.
For effectively problem-solving skills to collect relevant data about Mrs. Hatch I would ask
her to describe how she is feeling for me once again, I would listen attentively to her and repeat
what it is she is saying to me just to make sure I have heard her correctly. I would document
what it is she’s saying to her chart. I would then ask open-ended questions and report her
complain to an RN nurse or doctor. Some questions I would ask are:
 On a scale of 1-10 how bad is the pain in your upper abdomen?
 When was the last time you had something to eat? Or after waking up from surgery
were you given anything to eat or drink
 Did you receive any medication after waking up from surgery? If yes do you know
what it was.
 How long have you been feeling nauseous?
 How long has your abdomen felt distended?
 Have you consumed any fluids since having surgery?

With receiving all these answers along with my documentation, I would take this to the RN
or doctor to have them further assess Mrs. Hatch and figure out what would be our next step.

5. Implement appropriate interventions at a support worker level for this client who is nauseated.
As a support Worker, I would provide care fixing Mrs. Hatch frequent light meals
throughout the day; serving foods cool or at room temperature; avoiding fried foods; spicy foods;
sticking to bland foods, such as dry crackers. Also make sure Mrs. Hatch's mouth is kept clean. I
also would report immediately any signs such as if is there blood or material that looks like
coffee grounds in the vomit, report if the Mrs. Hatch cannot keep liquids or food down, if she is
feeling weakness or dizziness and also if severe stomach pains occur with vomiting.

6. Demonstrate effective communication techniques with the client, family, and health care team
at all times.
I would choose my words carefully, with simple everyday language, speaks clearly
slowly and distinctly control the volume and tone of my voice in order to be concise and present
the information in a logical manner as one question at the time. Always explain Mrs. Hatch what
I am doing before and while doing the tasks, take the time to ask the Mrs. Hatch if she has
questions about the information relayed, ensuring confidentiality at all times. For both Mrs.
Hatch and her daughter, I would demonstrate active listening, always showing sincere empathy
when provide emotional care. Into the health care team, I would share my experiences, opinions
and ideas in a professional way. Also, with supervisor I would keep Mrs. Hatch’s records,
reporting the daily care provide and report any signs of complication.
